Current onsite plant staff is sufficient for plant operation.
This event is being reported in accordance with 10 CFR 50.72(a)(1)(i) due to the declaration of an emergency classification as specified in the approved Emergency Plan.
There was no impact on the health and safety of the public or plant personnel. The NRC Resident Inspector has been notified.
The NRC decided to remain in the Normal mode of operation at 1320 EDT.

Unit 1 tripped following the loss of power and is stable in Mode 3. Unit 2 reduced power to 90 percent but was otherwise unaffected by this event. Offsite services responded to the switchyard fire. The fire was extinguished at 0925 CDT.
There is no radioactive release and no threat to public safety.

At 0702 CDT on 7/24/2024, with Unit 1 in Mode 1 at 100 percent power, the Unit 1 reactor automatically tripped due to loss of offsite power. The trip was not complex, with all systems responding normally post-trip. No equipment was inoperable prior to the event that contributed to the event or adversely impacted plant response to the scram.
Operations responded and stabilized the plant. Decay heat is being removed by steam generator power operated relief valves (PORV). Unit 2 was reduced in power to approximately 90 percent power due to conditions in the switchyard.
Due to the reactor protection system actuation while critical, this event is being reported as a four-hour, non-emergency notification per 10 CFR 50.72(b)(2)(iv)(B).
There was no impact on the health and safety of the public or plant personnel. The NRC resident inspector has been notified.

At 0702 CDT on 7/24/2024, with both Unit 1 and 2 in Mode 1 at 100 percent power, the South Texas Project (STP) north and south switchyard electrical buses were de-energized.
In Unit 1, all emergency diesel generators (EDGs) 11, 12, and 13 automatically started in response to loss of offsite power on train `A', `B', and `C' engineered safety feature (ESF) buses.
Also in Unit 1, trains `A', `B', and `C' of the auxiliary feedwater (AFW) system automatically started.
In Unit 2, EDG 22 automatically started in response to loss of offsite power on the train `B' ESF bus.
Also in Unit 2, train `B' of the AFW system automatically started.
This event is reportable under 10 CFR 50.72(b)(3)(iv)(A) as an event that resulted in the valid actuation of a pressurized water reactor auxiliary feedwater system (50.72(b)(3)(iv)(B)(6)) and emergency alternating current (AC) electrical power system (50.72(b)(3)(iv)(B)(8)). RPS ACTUATION
The following information was provided by the licensee via email:
On March 21, 2024, at 2328 CDT, Waterford 3 Steam Electric Station, Unit 3 was operating at 98 percent power when an automatic reactor trip was initiated due to a loss of offsite power to the B train and a failure to automatically transfer from unit auxiliary transformer B to startup transformer B.
Emergency feedwater actuation signal 2 (EFAS), safety injection actuation signal (ECCS), containment isolation actuation signal and emergency diesel generators automatically actuated. The unit is currently stable in Mode 3. All control rods fully inserted and all other plant equipment functioned as expected. Forced circulation remains with one reactor coolant pump per loop running. Decay heat removal is via the main condenser. A train safety bus is being supplied by off-site power, and B train safety bus is being supplied by emergency diesel generator B.
Following the loss of offsite power to the B train, it was reported that main transformer B and startup transformer B were both on fire. The Emergency Director declared an Unusual Event at time 2337 CDT. The fire was reported extinguished at 0009 CDT on March 22, 2024, and the Unusual Event was terminated at 0221 CDT on March 22, 2024. Offsite assistance was requested. The local fire department responded to the site but the fire was extinguished by the on-shift fire brigade. NRC Region IV management was contacted regarding the emergency plan entry at 0030 CDT on March 22, 2024.
This event is being reported as a 4-hour non-emergency notification in accordance with 10 CFR 50.72(b)(2)(iv)(B) as an actuation of the reactor protection system (RPS) when the reactor is critical and as an 8-hour non-emergency notification in accordance with 10 CFR 50.72(b)(3)(iv)(A) as valid actuation of the EFW system, ECCS, Containment Isolation and Emergency Diesel Generators.
The NRC Resident Inspector has been notified.

The initial notification in event notice #57042 by Waterford Steam Electric Station, Unit 3, reported a Notice Of Unusual Event (NOUE) emergency declaration due to a fire in the protected area requiring off site support to extinguish. The basis for retraction of the initial emergency notification is that this event did not meet the definition of a fire in the protected area that requires off site support to extinguish. Guidance provided in Nuclear Energy Institute (NEI) 99-01, Rev. 6 and implemented in Waterfords Emergency Plan procedure, initiating Condition HU4.4 states, The dispatch of an offsite firefighting agency to the site requires an emergency declaration only if it is needed to actively support firefighting efforts because the fire is beyond the capability of the Fire Brigade to extinguish. (NOTE: The Initial Notification Form sent from the Control Room at 2341 CDT on March 21, 2024, requested by and provided to the NRC Headquarter Operations Center via e-mail at 0302 CDT on March 22, 2024, stated that the Emergency Classification had been made on Initiating Condition HU4.4 rather than HU4.1)"
When the event occurred on March 21, 2024, the Emergency Director declared an Unusual Event at 2337 CDT and requested offsite support based on the information available at that time including the initial assessment by the fire brigade leader and expected need for offsite support to extinguish the fire. As reported in the 0420 EDT update on March 22, 2024, the fire was reported extinguished at 0009 CDT on March 22, 2024, by the Waterford Fire Brigade without the need of offsite support."

The basis for the update to the initial notification is that this event did not meet the definition of a Fire in the Protected Area that requires offsite support to extinguish. As provided in NEI 99-01, Rev. 6 and implemented in Waterfords emergency plan procedure, initiating condition HU4.4 states, The dispatch of an offsite firefighting agency to the site requires an emergency declaration only if it is needed to actively support firefighting efforts because the fire is beyond the capability of the Fire Brigade to extinguish. The remaining events that were reported in EN 57042 as a 4-hour non-emergency notification in accordance with 10 CFR 50.72(b)(2)(iv)(B) as an actuation of the reactor protection system (RPS) when the reactor is critical and as an 8-hour non-emergency notification in accordance with 10 CFR 50.72(b)(3)(iv)(A) as valid actuation of the EFW (emergency feedwater) system, ECCS (emergency core cooling system), containment isolation and emergency diesel generators are still applicable and require no additional update at this time.

Watts Bar Nuclear Plant (WBN) is retracting Event Notice 56809, Notice of Unusual Event, based on the following additional information, not available at the time of the initial notification.
Specifically, in accordance with the emergency preparedness implementing procedures, WBN reported a condition that was determined to meet emergency action level (EAL) HU4, Initiating criteria number 1, receipt of multiple (more than 1) fire alarms or indicators and the fire was within any Table H2 plant area, which includes the diesel generator building. It was further determined that multiple fire detection zones actuated (spurious and invalid) enabling the discharge of installed fire suppression (CO2) into the space. Upon entry by the site fire brigade, it was determined that no smoke or fire existed and reported to the Shift Manager at 0930 EDT. All fire alarms were reset. Troubleshooting activities are in progress to determine the cause. A fire watch has been established and CO2 has been isolated. The required compensatory measures for the affected areas will remain in place until completion of the investigation, and CO2 suppression is restored to functional.

At 1446 EST it was verified that the leak had been isolated. The plant remains at 100 percent power.

Turkey Point Nuclear Unit 3 is retracting this notification based on the following additional information not available at the time of the notification, Following the event for unidentified RCS leakage on 12/8/22 at 1403 EST, additional information demonstrated that an RCS leak did not exist.
A review of indications showed that the in-service seal water return filter D/P (Differential Pressure) rose from 1 psid to 6 psid with a corresponding rise in VCT (Volume Control Tank) level as would be seen with an increase in excess letdown flowrate. Additionally, the on-contact radiation dose rate from the excess letdown piping in the U3 Pipe and Valve Room was measured by RP (Radiation Personnel) to be significantly higher than normal with no other sources of elevated radiation levels noted.
Following the closure of CV-3-387, RCS To Excess Letdown HX (Heat Exchanger) Control Valve, a nominal input/output flow balance was able to be restored. Containment parameters including pressure, temperature, sump level and radiation level did not change during the event. Due to the shared nature of piping between excess letdown and seal water return, a challenge to system integrity would not allow RCP (Reactor Coolant Pump) controlled bleed-off to remain in service with a normal flow balance which it has at all times subsequent to the event.
Turkey Point Nuclear reported initially based on the available information at the time and to ensure timeliness with emergency declaration and reporting notification requirements. The NRC Resident Inspector has been notified.

Additionally, security in a tower heard an explosion. Both units remain in Mode-1, 100 percent and they are investigating the validity of the seismic alarms before proceeding with the Abnormal Operating Procedure required shutdown.

On 4/29/22, at 0406 EDT, Sequoyah Unit 1 and Unit 2 terminated the Notice of Unusual Event. The Civil Engineers determined that the alarms were due to a failed seismic indicator channel. Through interviews, only one security officer felt ground movement for a couple of seconds and heard a faint rumbling sound.

Subsequently, the fire was extinguished at 1402 CDT. Unit 3 remains in Mode 1 at approximately 9.5 percent rated thermal power (RTP). Unit 1 and 2 remain at 100 percent RTP and unaffected.

The fire began at 1332 CDT. It is believed that the fire was in the oil system of the Feedwater Pump. The fire was extinguished by the on-site fire brigade. No off-site assistance was requested. The Unusual Event was declared under Emergency Action Level HU-4. The licensee notified the NRC Resident Inspector and required State and local government agencies. Unit 3 is currently stable.

At 0343 EST on February 5th, 2022, with Unit 2 in Mode 1 at 100% power, the reactor automatically tripped due to Reactor Coolant Pump (RCP) Flux/Flow Imbalance caused by the simultaneous trip of all 4 RCPs. The cause of the loss of all RCPs is under investigation. The trip was not complex. There was no inoperable equipment prior to the event that contributed to it.
Operations responded and stabilized the plant. Decay heat is being removed by the Reactor Coolant System (RCS) in natural circulation with normal feedwater flow.
Due to the Reactor Protection System (RPS) actuation while critical, this event is being reported as a four hour, nonemergency notification per 10 CFR 50.72(b)(2)(iv)(B).
There was no impact on the health and safety of the public or plant personnel. The NRC Resident Inspector has been notified.

The follow-up investigation determine that the cause of the trip was determined to be a failed fuse in an undervoltage monitoring circuit for the electrical bus powering the reactor coolant pumps. The trip of the reactor coolant pumps resulted in an automatic reactor trip as designed. The plant responded as expected. There was no fire. The response of the main feedwater system caused an expected increase in temperature in a standby portion of the system which caused the breakdown of the pipe coating producing smoke. There was no release to the environment. The reading of a single instrument was determined to be invalid and other indications supported the conclusion that no release occurred.

All rods fully inserted during the scram. The plant electrical line up is both SBDGs (Standby Diesel Generators) are running.
Decay heat is being removed via SRVs (Safety Relief Valves) to the torus.

The unit was at approximately 20 percent power and was not synced to the grid when the unit automatically scrammed. All control rods fully inserted. Emergency Diesel Generators started and began powering the safety buses. Safety systems actuated as expected.
The Unit also experienced a loss of Fuel Pool Cooling and Cleanup System, but one pump was returned to service.
Unit 2 remains at 100 percent power and is unaffected.
The licensee notified State and local governments, as well as the NRC Resident Inspector.

At approximately 2302 EDT, a loss of offsite power occurred on Unit 1. This resulted in a Reactor Protection System (RPS) actuation. Per design, emergency diesel generators 1 and 2 properly started and loaded to their respective emergency buses. The Reactor Core Isolation Cooling (RCIC) system was manually started and is being used to control reactor water level. The High Pressure Coolant Injection (HPCI) system was manually started and is being used for pressure control. As previously reported, an Unusual Event was declared at 2312 EDT due to the loss of offsite power. At the time of the event, Unit 1 was in the process of shutting down for maintenance associated with a ground on the main generator.
Due to the RPS actuation while critical, this event is being reported as a four-hour, nonemergency notification per 10 CFR 50.72(b)(2)(iv)(B).
As a result of the reactor trip, reactor water level reached low level 1 (LL1). The LL1 signal causes a Group 2 (i.e., floor and equipment drain isolation valves), Group 6 (i.e., monitoring and sample isolation valves) and Group 8 (i.e., shutdown cooling isolation valves) isolations. The LL1 isolations occurred as designed; the Group 8 valves were closed at the time of the event. Per design, the loss of offsite power also caused a Group 1 (i.e., main steam isolation valve) isolations.
Due to the Emergency Diesel Generator and Primary Containment Isolation System (PCIS) actuations, this event is also being reported as an eight-hour, nonemergency notification in accordance with 10 CFR 50.72(b)(3)(iv)(A).
Unit 2 was not affected. There was no impact to the health and safety of the public or plant personnel. The safety significance of the event is minimal. All safety related systems operated as designed.
Investigation of the cause of the loss of offsite power is in progress.

At 1454 EDT on August 4, 2020, the Unusual Event was exited when offsite power was restored to Unit 1.
Per design, when the loss of offsite power to Unit 1 occurred, all four emergency diesel generators (EDGs) started and EDGs 1 and 2 properly suppled emergency buses 1 and 2. Since Unit 2 was not affected by the loss of power, EDGs 3 and 4 ran unloaded.
With restoration of offsite power to Unit 1, EDG 2 has been secured. EDGs 1, 3, and 4 are being secured as required by plant operating procedure.

As stated, the reactor was in a shutdown condition at the time of the event. There was no effect on the reactor due to the flood water.

At 1308 EDT the fire was declared out and fire watches posted.
Offsite assistance was requested during the event and the Jenkinsville, SC fire department responded to the site. There were no plant personnel injuries or impact to the health and safety of the public.
The cause of this event is unknown at the present time. The electrical bus has been de-energized. The unit is currently in a planned refueling outage.

Watts Bar Nuclear Plant (WBN) is retracting Event Notice 54531 (NOUE notification) based on the following additional information.
WBN reported a condition that was determined to meet the definition of a FIRE in the plant Emergency Preparedness Implementing Procedures (EPIP) based on indications available to the decision-maker at the time the declaration was made. A fire, without observation of flame, is considered present if large quantities of smoke and heat are observed.
Moderate quantities of smoke were observed coming from an electrical cabinet not required to support safe plant operation. Once Fire Brigade personnel were able to access the affected room, no evidence of flame or significant heat was observed. Plant personnel ultimately determined that an overheated electrical component (transformer) resulted in the smoke. As such, the actual conditions did not meet the EPIP definition of a fire.
The NRC Resident Inspector has been notified of this retraction.

Technical Specification Action 3.4.4.A, Unidentified Reactor Coolant System (RCS) leakage increase not within limit, requires RCS leakage to be reduced to within limits within 8 hours.
It is expected that the leakage would not have been reduced to within limits within the required Technical Specification
completion time; therefore, this event is being reported in accordance with 10 CFR 50.72(b)(2)(i).
There was no impact on the health and safety of the public or plant personnel.

At 0546 CDT, Cooper Nuclear Station declared an Unusual Event due to the Missouri River level reaching 899.05 feet above mean sea level (MSL), which is above the Emergency Action Level (EAL) HU 1.5 elevation of 899 feet above MSL. The river is expected to crest above 901 feet above MSL within the next day, and remain above 899 feet above MSL for the next several days.
Declaration of an Unusual Event is a 1 hour report, and is reportable under 10 CFR 50.72.a.1.1. Actions are in progress in accordance with site flooding procedure, including strategic placement of barriers at building entrances and important facilities. There is no major plant equipment out of service at this time. Personnel access to the site is not presently impeded and emergency evacuation routes remain available.
A press release is planned for this event, which is a four hour report, reportable under 10 CFR 50.72.b.2.11.
If the Missouri River were to reach 901.5 feet above MSL, Cooper would initiate a unit shutdown in accordance with their procedures. If the Missouri River were to rise greater than 902 feet above MSL, Cooper will declare an Alert.

The site determined that the alarm was invalid and terminated the NOUE. Unit 2 is in a stable condition and in a normal electrical lineup. Offsite support was not requested.

At 1036 EDT, Sequoyah Nuclear Plant (SQN) terminated the Notification Of Unusual Event (NOUE) due to initial report of heat and smoke in Unit 2 Lower Containment.
At 1000 EDT, it was determined that no fire had occurred. Due to difficulty of access to some of the areas being searched, the source could not be identified prior to 1000 EDT. No visible flame (heat or light) was observed.
The source of the smoke was determined to be residual oil from a hydraulic tool oil in contact with pressurizer piping. The pressurizer piping was being heated up to support Unit 2 start-up following U2R22 refueling outage. Once the residual oil dissipated, the smoke stopped. It has been concluded that no fire or emergency condition existed.
Unit 2 is currently in Mode 5, maintaining reactor coolant temperature 160F-170F and pressure 325psig-350psig with 2A Residual Heat Removal (RHR) system in service in accordance with U2R22 refueling outage plan.

Following a full Reactor Building inspection, it was concluded that a fire did not exist. The source of the smoke originally reported was later determined to be residual oil from a hydraulic tool in contact with pressurizer piping. Once the residual oil dissipated, the smoke stopped. The source of heat originally reported was normal heated conditions associated with the pressurizer commensurate with plant conditions. SQN reported initially based on the available information at the time and to ensure timeliness with emergency declaration and reporting notification requirements.

At 1748 CDT Unit 1 received a call reporting smoke coming from the 480V Condensate Demineralizer Panel 3 in the Unit 3 turbine building elevation 557'. At approximately 1803 (CDT), the incident commander on the scene confirmed a fire inside the panel and all three units entered 0-AOI-26-1, Fire Response. The board was subsequently de-energized by operations personnel and the fire was extinguished at 1806 CDT. SM (Shift Manager) exited EAL HU4 and all three units exited 0-AOI-26-1 at 1840 CDT.
Fire operations remain on scene to monitor. A team is being assembled for damage assessment and recovery.

The Notification of Unusual Event was terminated at 1025 CDT.
The toxic gas release occurred when site personnel were filling a fire suppression carbon dioxide (CO2) tank outside the diesel generator building. The relief valve in the common diesel generator room for Unit 1 and 2 diesel generators inadvertently lifted causing a toxic gas environment by releasing CO2 into the room. The licensee terminated the tank fill stopping the release of CO2, and with the door to the room being opened, the gas cleared in about 20 minutes.
The licensee has notified the NRC Resident Inspector.

The total loss of 4160 volt AC power was for approximately 30 seconds. The unit is refueled and reactor reassembly complete up to bolting on the reactor head. There was no effect on Units 1 and 2.

The fire was declared out at 1119 (EST), 2/1/18. The fire was caused when steam leak repair injection equipment failed and leaked onto hot piping. There was no equipment damage or impact to plant operation. The fire was extinguished by the fire brigade. Offsite assistance was not required.
The fire resulted from Furmanite repair of a Moisture Separator Reheater inlet flow control valve. The unusual event will be terminated when sufficient lagging is removed to verify the extent of leaked fluid.

The leak was reported to be caused by Maintenance cutting into a one inch line. The total size of the leak is unknown, however, it is estimated to be depressurizing in the main generator at approximately 1lb per hour. The current pressure is 52 to 53 lbs. pressure and is stable. The operations staff have entered their abnormal procedure and are taking actions to isolate the leak. Operators have isolated the source of hydrogen and have opened the exterior roll up doors to increase the airflow and minimize the concentration of hydrogen in the area. The area has been evacuated and hot work has been stopped.

On 11/14/2017, Nebraska Public Power District will issue a press release concerning the declaration of a Notification of Unusual Event (EN#53069) declared today at 1118 (CST) for a small hydrogen leak in the turbine building.
This is a four hour report per 10CFR50.72(b)(2)(xi) for any event or situation for which a news release is planned or notification to other government agencies has been or will be made which is related to heightened public or government concern.
