r Attachment to U-602049 CLINT04 POWER STATION NOTIFICATION OF UNUSUAL EVENT on Tuesday, September 22, 1992, at approxitstely 0915 hours0.0106 days <br />0.254 hours <br />0.00151 weeks <br />3.481575e-4 months <br />, a Notification of Unusual Event (NOUE) was declared in accordance with Emergency Plan Implementing Procedure EC-02 Emergency Classifications.

This procedure requires that a Notification of Unusual Event be declared when a potentially contaminated, injured person is transported to r.n offsite medical facility for treatment.

Clinton Power Station was operating at approximately 100% power in Mode 1. At approximately 0855 hours0.0099 days <br />0.238 hours <br />0.00141 weeks <br />3.253275e-4 months <br />, the Control Room received notific.ation that an employee aorking in the radwaste sort tent had been injured and was potentia 11) ontaminated, and that an ambulance had been called to respond, At approximately 0915 hours0.0106 days <br />0.254 hours <br />0.00151 weeks <br />3.481575e-4 months <br />, the ambulance departed the site with

! the victim and the Shif t Supervisor declared the Notification of Unusual

Event. The State of Illinois was notified at 0928 hours0.0107 days <br />0.258 hours <br />0.00153 weeks <br />3.53104e-4 months <br /> and the NRC Incident Response Center was notified i.t 0958 hours0.0111 days <br />0.266 hours <br />0.00158 weeks <br />3.64519e-4 months <br />.

At approximately 0955, the Shift Supervisor received a report from the hospital that the victim was not contaminated, At approximately 1020, the Shift Supervisor received a report that the ambulance and hospital were surveyed for contamination and no evidence of contamination was found; therefore, the Shif t Supervisor terminated the Notification of Unusual Event.
