Pzga 1 of 4 Attachment to U-601420 CHALLENGES TO MAIN STEAM SAFETY / RELIEF VALVES Month March 1989 On March 19, 1989, Safety Relief Valves (SRVs) IB21-F047A, 1B21-F047D, 1B21-F051C and 1B21-F051D opened to relieve a reactor pressure increase caused by normal control rod drive " bypass" flow following a reactor scram. The reactor was undergoing a pressure test (under solid water conditions) of 1000 psig to check for reactor coolant pressure boundary leakage. Single control rod scram time testing was being performed while the reactor was pressurized to the designated test pressure. Upon completion of the control rod testing, the reactor mode switch was required to be repositioned from " Refuel" to " Shutdown." Placing the reactor mode switch in the " Shutdown" position initiates a reactor scram signal. This condition was anticipated and appropriate plans for lowering reactor pressure (to 900 psig) were made and implemented.

However, with the reactor at " solid conditions" (i.e., no volume addition without a significant increase in pressure), a control rod drive pump running, and a planned reactor scram (which increased control rod drive bypass flow), reactor pressure increased (approximately 7 psig/second) to the relief setpoint of the subject SRVs. Additional information will be provided in LER 87-016.

These figures will be used to plot a graph fcr each reporting month. Note that when maximum dependable capacity is used for the net electrical rating of the unit, there may be occasions when the daily average power level exceeds the 100% line (or the restricted power level line). In such cases, the average daily unit power output sheet should be footnoted to explain the apparent anomaly.
