January 21, 2003 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE -- PNO-III-03-002 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. The information is as initially received without verification or evaluation, and is basically all that is known by the Region III staff on this date.

REACTOR TRIP DUE TO MAIN TRANSFORMER FIRE DESCRIPTION:

At about 8:10 p.m. (EST) on January 15, 2003, a fault in the Unit 1 main power transformer resulted in a fire, which caused a trip of the main turbine and an automatic reactor trip. All control rods fully inserted. The reactor coolant pumps are running with offsite power available. Offsite fire assistance was called to the plant but was not needed because the plants fire brigade extinguished the fire by 8:35 p.m. (EST) prior to their arrival.

One individual received a minor injury due to a fall and smoke inhalation and was transported to a local hospital for medical attention. The individual was treated and released from the hospital. The fire also resulted in a transformer oil spill, which the licensee is in the process of cleaning up.

The licensee declared an UNUSUAL EVENT at 8:25 p.m. (EST), due to a fire in the protected area that was not extinguished within 15 minutes. The UNUSUAL EVENT was terminated at 10:20 p.m. (EST). NRC Region III (Lisle, IL) and Headquarters Offices monitored the situation.

Appropriate state and local officials were notified of the event and the resulting transformer oil spill.

The licensee is investigating the cause of the transformer fire. The resident inspectors are monitoring the licensees investigation, maintenance, and repair activities. The information in this preliminary notification has been reviewed with licensee management.

A one-hour notification report was made to the NRC Headquarters Operations Officer on January 15, 2003 at 9:01 p.m. (EST). A notification update was made on January 15, 2003 at 11:39 p.m. (EST). This information is current as of January 21, 2003 at 8:00 a.m. (CST).
