Revision 09, This JPM is developed IAW guidelines established in NUREG 1021 Rev 8 ES-301 and Appendix C. This JPM meets the criteria of Category B.1 "Control Room Systems," for RO/SRO candidates.

Revision 10, JPM revised to reflect procedure changes.

Revision 11, JPM revised to reflect changed plant equipment and procedure.

Revision 12, JPM modified for use on the 2021 ILT NRC Exam.

2021 ILT NRC JPM a SIMULATOR SETUP INSTRUCTIONS

1. 1.Reset the simulator to IC 18 .

NOTE: It is okay to use a similar IC to the IC listed above, provided the IC actually used is verified to be compatible with this and other JPMs that are scheduled to be run concurrently. {Delete this NOTE if not applicable.}

2. Do the following panel actions o Trip the B ASD by closing MO 1-0202-5B valve o Verify 1-0202-4B valve is Open with the C/S in NEUTRAL o Verify both RR controllers are in manual o Place DFWLC in single element o Adjust A ASD speed to 42%

o Verify no ASD Speed Hold exists o Perform QCOP 0202-43 steps F.6.b. thru i. and F.6.k. thru n.

o Acknowledge and clear annunciators and OWS alarms.

3. Insert control rods to establish a 51% FCL.
4. Prepare a copy of QCOP 0202-43 marked up through step F.9.
5. Provide a calculator.
6. When the above steps are completed for this and other JPMs to be run concurrently then validate, if not previously validated, the concurrently run JPMs using the JPM Validation Checklist.
7. This completes the setup for this JPM.

Investigation revealed the trip occurred when a contractor inadvertently bumped the breaker at Bus 12 while moving scaffold.

Bus 12 has been inspected by Electrical Maintenance and has been cleared for operation.

All actions have been completed for single loop operation per QCOA 0202-04 and QCOP 0202-07.

The unit is currently operating on the 51% FCL.

A QNE is in the control room and is monitoring thermal limits per Attachment C.

An EO and the System Engineer are stationed at the 1B ASD with a field copy of QCOP 0202-43.

An EO is stationed at Rack 2251-38 to monitor oil level for the Recirc pump start.

The ASD B Cooling System is ON and conductivity is < 1µS/cm.

The Steam Dome to Bottom Head coolant differential temperature was determined 2 minutes ago at 92.2ºF.

The Recirc operating and idle loop differential temperature was determined 1 minute ago at 2ºF.

INITIATING CUE Start the 1B ASD per QCOP 0202-43, REACTOR RECIRCULATION SYSTEM STARTUP, Step F.10. through F.13.

Provide Examinee with: Marked up copy of QCOP 0202-43 and Attachment B.

Fill in the JPM Start Time when the student acknowledges the Initiating Cue.

Post scram actions are in progress.

Reactor water level is at +30 inches and rising.

Both RWCU Demins are isolated and the remote manual inlet and outlet valves are closed. (RM 1-1279-13A, RM 1-1279-13B, RM 1-1279-26A, and RM 1-1279-26B)

RBCCW system is in operation No adjustment has been, or needs to be made to the NRHX high temperature Isolation setpoint.

INITIATING CUE Start the RWCU system in Reject Mode at approximately 150 gpm per QCOP 1200-07, RWCU SYSTEM COOLANT REJECTION, Attachment A.

2021 ILT NRC JPM g INITIAL CONDITIONS The Unit is in Day 3 of 7 for a Maintenance outage.

QCIS 0700-09, NEUTRON MONITORING FUNCTIONAL TESTS WITH REACTOR MODE SWITCH NOT IN RUN, has been completed by Instrument Maintenance.

Instrument Maintenance has completed the troubleshooting package for the SRM 21 downscale failure and recommended the detector be replaced.

  • The carousel is stowed, locked and grating removed.
  • The Maintenance crew has verified a clear travel path for the detector and cabling and are in place to observe SRM 21 withdrawal.

The Work Instructions require SRM 21 to be fully withdrawn and taken out of service.

INITIATING CUE Withdraw SRM 21 to the full out position per QCOP 0700-01,SOURCE RANGE MONITOR (SRM) OPERATION, step F.4.

Provide examinee with: A marked up copy of QCOP 0700-01.

Fill in the JPM Start Time when the student acknowledges the Initiating Cue.

2021 ILT NRC JPM k INITIAL CONDITIONS A loss of off-site power has occurred on Unit 1.

The 1/2 EDG failed to auto-start, a manual start from the Control Room was attempted but was not successful.

QCOA 6600-02 has been entered and Operators are taking actions directed by the procedure.

The 1/2 EDG is in its normal standby line-up with the output breaker open as verified by the Control Room and locally by the Equipment Operator.

Per QCOA 6600-02, an HGA Relay block was placed on ASR 1/2-1 in Bus 13-1, cubicle 1 but the 1/2 EDG did not start.

An Equipment Operator is standing by to perform the following tasks:

  • Verify the 1/2 EDG output breaker closes when required.
  • Locally load the 1/2 EDG as directed by the Control Room.

INITIATING CUE Locally start the 1/2 EDG in accordance with QCOP 6600-11, DIESEL GENERATOR LOCAL OPERATION, to provide power to Bus 13-1. Notify the Control Room when complete.
