Unit 1 and Unit 2 are in Mode 1.

There is a severe Thunderstorm warning in effect for the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> for Clinton, Whiteside, and Rock Island counties with expected rainfall in excess of 4 inches.

A Tornado warning has also been issued for Clinton, Whiteside and all of Rock Island counties.

- Verify RWM indication is ?? for M -10 and annunciator 901-5 B-3, ROD WORTH MON BLOCK, is in alarm.

- Verify position 36 is displayed for control rod M -10 on the Full Core Display

- Deselect the control rod and clear annunciator 901-5 B-3.

INITIAL CONDITIONS You are the NSO.

During the previous shift, annunciator 901-5 B-3, ROD WORTH MIN BLOCK, alarmed and the NSO reported position indication on the RWM for control rod M -10 was lost.

Instrument Maintenance and the QNE were notified.

Alternate position indication is available on the Full Core Display and a digital readout in the Aux Electric Room at the 901-27 panel per IM Work Package #0037465.

After troubleshooting, Instrument Maintenance and IT has informed the Shift manager that full indication on the RWM can be restored in approximately 10 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br />.

The QNE has requested that control rod remain at position 36 with a substitute position installed on the RWM.

3999-542, SERVICE WATER RADIATION MONITOR INLET ISOLATION VALVE is verified open.

- EDUCTOR INLET PRESSURE is 20 psig.

INITIAL CONDITIONS

Unit 1 was operating at 100% rated power when a HPCI steam line ruptured occurred during a scheduled surveillance run. The Emergency Plan was activated and a SITE AREA EMERGENCY (FS1) was classified at time 1300.

QGA 100 and QGA 300 have been entered by the Unit Supervisor.

All plant personnel have been notified of the classification level, reason for the classification, and the TSC and OSC have been activated.

All attempts to isolate HPCI Steam Line isolation valves MO 1-2301-4 and MO 1-2301-5 have been unsuccessful.

The plant conditions at the time of classification were:

  • RPV water level 30 inches
  • Drywell Pressure 1.5 psig
  • Torus Pressure 0.5 psig
  • HPCI room temperatures 180 °F
  • Reactor Building Vents are isolated and 1/2B SBGTS is running at 4000 scfm.
  • Reactor Building D/P is +3.0 H2O as indicated on DPI 1-5740- 22 at the 912-5 panel.
  • No increases have been reported in Main Chimney Effluent radiation levels.

You are an extra SRO in the Control Room and the Shift Manager has requested a verification of the NARS form prior to transmission.
