Attachment Inoperable Instruments (Period Covered January 1987 through June 1987) 1.
Containment Ventilation Unit Condensate Drain Tank (CVUCDT) Continuous Composite Samplers, Units 1 and 2.
On June 1, 1984 both CVUCDT Composite Samplers were declared inoperable due to their inability to dependably sample the waste stream. The problem was fouling due to algae growth within the tank. Nuclear Station Modifications were written to replace the samplers with equipment more compatible with the waste stream. The modifications resulted in the thirty (30) day inoperability limit as stated in Technical Specification 3.3.3.8, Table 3.3-12, Action 33 being exceeded.
Following installation and periods of testing both CVUCDT Composite l
Samplers were declared operable. Unit one (1) was declared operable on February 27, 1987 and Unit two (2) was declared operable on May 26, 1987.

Radwaste Facility Ventilation System Flow rate Monitor.
On 11-04-86 the RWF flow rate monitor was declared inoperable by McGuire I&E personnel.
This period of inoperability exceeds the thirty (30) day limit as stated in Technical Specification 3.3.3.9, Table 3.3-12, Action 36.
During periodic maintenance by I&E the system flow transmitter (MVAFT 5300) was found to contain moisture, originating from the VI air supply line, the flow transmitter was replaced and calibrated. However, the transmitter could not be functionally tested due to the RWF ventilation being secured.
On March 27, 1987 I&E was able to complete the functional test of the flow transmitter subsequently declaring the instrument operable on that date.

Unit One (1) Containment Ventilation Unit Condensate Drain Tank Flow Rate Monitor.
I On 10-12-86 the CVUCDT flow rate monitor was declared inoperable.
Inoperability was due to the replacement of the Flow Element (1FLFE-5900) and Flow Transmitter (1WLFT-5900).
This replacement was performed in l
conjunction with the installation and calibration of the new CVUCDT composite samplers, (see item number 1).
The equipment was installed and removed from the Technical Specification
