HIGHLIGHTS OF WEEKLY INFORMATION REPORT

. WEEK ENDING DECEMBER 12, 1986 Grand Gulf Station. Unit 2 Middle South Utilities, Inc. (MSU), a public utility holding company for several operating subsidiaries, including Mississippi Power & Light Company, issued a press release December 5, 1986, regarding the suspension of construc-tion of Grand Gulf. Unit 2. The Boards of Directors of MSU and of Systems Energy Resources. Inc. (formerly named Middle South Energy Inc., another subsidiary company of MSU), have decided to continue the suspension of con-

'struction on Grand Gulf Unit 2 for a period of up to 3 years. The decision of the Boards was based on a special study team report which assessed various alternatives. The decision keeps open the alternative of completing Grand Gulf Unit 2, should economic growth of the region result in increased energy needs. This means that the various equipment maintenance activities to assure continued licenseability of Unit 2 will be undertaken during this period of suspension.

Grand Gulf. Unit 2 is about 34% completed in terms of project man-hours and represents an investment of $947 million as of September 30, 1986.

Duane Arnold Energy Center At 1412 CST, December 9, 1986. DAEC manually started reducing power to go to cold shutdown. The shutdown is being made because the licensee determined that the Thomas and Betts (nylon insulated) control cable splices presently installed in drywell junction boxes may be environmentally unqualified. This determination was made by the licensee after verbal information was received on tests made by Wylie Labs on similar splices. This infomation indicated that the present licensee calculated " worst case accident" scenario appears to be outside the envelope used in the tests. Some of the present splices will be replaced with environmentally qualified AMP splices and some will be repaired with environmentally qualified tape during the outage which is expected to last up to 10 days depending on the number of each upgrade.

Incinerator Incident at Savannah River Plant An incident occurred in the S area of the Savannah River Plant involving a test of a fire protection system, at about 10 to 11 a.m. on Tuesday, December 9.

Apparently the halon fire suppression chemical entered an active incinerator system and degraded, causing four workers to rece.ive toxic chemical exposures -

that required hospital treatment. All four were treated and released.

Radioactive materials involved were insignificant. As a result of the incident, workers on DOE's Defense Waste Processing Facility (DWPF), presently under construction, were evacuated, but returned to the area after 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br />.

Briefing to OMB on H?W Licensing Support System OMB received a request from DOE to approve a proposed contract to develop multiple licensing support systems (individual systems in the field and in Headquarters). As background prior to meeting with DOE, OMB asked that we brief them on the Licensing Support System. On Wednesday, December 10, 1986, a briefing and demonstration of our Pilot Project was held for OMB staff by NMSS staff. OMB was very complimentary to our staff for their successful efforts and quite impressed with our system. It appears that we are leading the field in this area and that our full text search / optical disc Pilot Project is a first of its kind.

l 0FFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending December 12, 1986 Calvert Cliffs Unit 1 During an ultrasonic inspection on December 3,1986 at Calvert Cliffs Unit 1 l which is down for refueling, an indication was found adjacent to a field weld '

in the 34" dia, 1" well main steam line. That indication showed a 1" wide, 24" long circumferential wall thinning i" downstream of the centerlir.e of the field weld. This indication was radiographed and the radiograph was compared to the radiograph made during construction. The construction radiograph showed

, a similar indication. The ultrasonic inspection showed the approximate depth of the thinning to be .14". BG&E personnel believe that the thinning has been in the pipe since construction and was caused by a mismatch during welding.

BG&E personnel have done calculations to show that the condition is not unsafe.

During a telecon on December 4,1986, they briefly outlined what calculations they had done to support their conclusions that no repair or replacement is necessary and that delay in their projected restart date of December 24, 1986 is not necessary.

Staff personnel requested that'BG&E personnel come to Bethesda Wednesday, December 10, 1986, to present the calculations supporting their conclusions.

Edwin I. Hatch Nuclear Plant, Units 1 and 2 As a result of loss of air supply to an inflatable seal in the transfer canal between the fuel pools for Hatch Units 1 and 2, a substantial amount of water .

leaked from these fuel pools. Some of this was discharged through storm drains to a swampy area within the owner-controlled property. The amount that leaked from the pool may have been as much as 140,000 gallons with 5,000 to 10,000 gallons having reached the swampy area. The licensee has constructed a number of earth fill dams to contain the spill water in the swampy area.

The activity of the release at the storm drain discharge was initially reported on 12/4/86 as 1.26 times the maximum permissible concentrations (mpc) for cesium-134, cesium 1-37, zinc-65, and manganese-54. By 12/5/86 the activity was well below mpc.

The event was caused by the erroneous closing of the isolation valve in the air supply line at 2200 CST on 12/2/86. The licensee did not discover that the air supply valve was closed until about 2200 CST on 12/3/86. The licensee >

notified the NRC at about 0130 on 12/4/86. Region II dispatched an Augment Inspection Team on the morning of 12/4/86. Unit I was operating at full power and Unit 2 was making final arrangements for restart following a refuel-ing outage. Unit I remains at full power and Unit 2 is still preparing for

NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT FOR DECEMBER 8 - 14, 1986

1. DEFUELING

- Efforts continued to remove the broken drill strings imbedded in the debris pile as a result of core drilling operations. Four of the six broken strings have been removed. Efforts will resume to remove the reaining two when water clarity is regained.

- Hydrogen peroxide was added to the Reactor Coolant System (RCS) to control the regrowth of the bio-organisms. The addition resulted in severe degradation of water clarity.

- A decontamination experiment was performed in the reactor building basement. A high pressure hydrolazer mounted on a robot was used to scarify the concrete. The results of the experiment are under evaluation.

- The licensee completed the first of a series of tests of a program that involve addition of coagulant chemicals to the Defueling Water Cleanup System (DWCS) process stream. The coagulant, in conjunction with a filter aid material, was added while processing water from a bleed tank through a DWCS filter canister. Preliminary test results indicate a substantial increase in DWCS filter life as well as very good water clarity on the filter effluent. The test program will continue.

2. ENVIRONMENTAL MONITORING

- US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and the City of Lancaster Agreement.

- TMI water effluents are sampled from the station (Units 1 and 2)

EPA. Gamma spectrum analyses of the seven discharge daily and analyzed composited samples by(November 22 - 29, 1986) indicated no TMI-2 related radioactivity.

- EPA's gamma spectrum analysis of the NRC's TMI outdoor air sample for December 4 - 11, 1986 showed no reactor related radioactivity.

- The Lancaster water works intake composited seven daily samples (November 23 - 29, 1986) gamma spectrum analyzed by EPA showed no reactor related radioactivity.

3. AUXILIARY AND FUEL HANDLING BUILDING ACTIVITIES

- Reclamation of Defueling Water Cleanup System (DWCS) filters continues.

- Fuel handling building ventilation system modifications are complcte. Testing is in progress.

- Pre-operational testing of the Sediment Transfer System revealed that the system will require modification before it can be operated.

Engineering studies are in progress to modify the system.

- Fuel canister preparation for shipment is in progress.

- Installation of an ultraviolet light / ozone device for water treatment has been placed on hold for industrial safety studies.

5. MEETING

- The Advisory Panel for the Decontamination of Three Mile Island Unit 2 met on December 10, 1986 in Harrisburg, PA. At this meeting the Panel was briefed by the licensee on their recent proposal for the Post Defueling Monitored Storage of TMI-2.

The next meeting of the Panel is tentatively scheduled for January 21, 1987 in Lancaster, PA. The exact date, time, location and agenda will be confirmed at a later date.

DECEMBER 12, 1936 ENCLOSURE B

l OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending December 12, 1986 Shieldalloy Corporation On December 3-4, 1986, members of NMSS, Region I, and the New Jersey Department of Environmental Protection met with representatives of Shieldalloy. The purpose of the visit was site familiarization and to discuss additional information needed to conouct the environmental assessment related to Shieldalloy's license renewal.

IAEA Activities During the week of November 17-21, 1986, Mr. Philip Robinson of the Division of Safeguards participated in the 12th IAEA Safeguards Seminar on Accounting and Reporting in Vienna, Austria. During the seminar, various countries detailed their experiences and problems associated with implementing IAEA safeguards.

The following week, November 24-29, Mr. Robinson participated in the US/IAEA Working Group on Accounting and Reporting. Mr. Robinson is a member of the working group as well as the coordinator. This working group is charged with the responsibility of identifying and correcting technical problems associated with reporting data to the IAEA.

Sheffield Illinois LLW Disposal Site Closure U.S. Ecology and the State of Illinois met on December 10 to resolve the few remaining areas of difference. Largely, these areas are institutional concerns between U.S. Ecology and the State. Further negotiations, which will include the NRC staff, will be held after the ASLB rules on the two outstanding legal issues. (The two legal issues are: (1) U.S. Ecology's possession of the buried waste; (2) can U.S. Ecology unilaterally terminate the license.)

DECEMBER 12, 1986 ENCLOSURE C

2 Barnwell Representatives of NRC's Division of Waste Management are scheduled to meet l with the South Carolina Department of Health and Environmental Control in Columbia, SC, on December 15-16, 1986. The purpose of the meeting is to ,

discuss NRC-DHEC activities as they relate to the Barnwell LLWDF as well as environmental and other data on site performance.

, Guidance on 20.302 Disposal by Subsurface Burial NMSS has recently completed detailed guidance (NUREG-1101, Vols. 2 and 3) for materials licensees on disposal by subsurface burial pursuant to Section 20.302 of 10 CFR 20. Copies are being sent to all materials licensees and agreement states. Guidance to reactor licensees is in preparation.

3 Superfund Action At Maxey Flats, KY, Low-Level Waste Site EPA has notified the State of Kentucky; U.S. Ecology, the former site operator; and approximately 800 waste generators and shippers that the agency is taking action at the site under the Comprehensive Environmental Response, Compensation and Liability Act as amended (CERCLA or Superfund). EPA has spent public funds investigating the situation and is considering additional action to determine what corrective actions should be taken. The letters to potentially responsible parties notify them of their potential liabilities to fund studies and corrective action and invites them to participate by forming a steering committee. The notices state that $130,000 has been spent, studies should cost $1,300,000, and cleanup may cost $30 million or more.

EPA action is based on off-site releases above background of tritium, cobalt-60, and strontium-90. Organic chemicals in trench leachate samples (e.g., benzene) were also mentioned. Part of the proposed study is to further define the nonradiological source terms. The proposed study recognizes the extensive studies that have been done and proposes to build on them. Under CERCLA, EPA is required to look at the no action and exhumation alternatives '

but expressed the view that neither was likely a reasonable choice based on both health and safety and costs. The proposed study will focus on risk analysis of current and potential releases of both radioactive and nonradioactive materials.

The Maxey Flats site accepted' wastes from 1962 to 1977. It was initially and continues to be licensed by the State and is currently owned and maintained by the State. NRC's only direct involvement is through the Office of State Programs. We can expect inquiries from licensees and increased concerns by States over the potential for second guessing under CERCLA and potential liability as site owner. sh s

Office of Inspection and Enforcement Items of Interest Week Ending December 12, 1986

1. Items of Interest
a. Surry Synopsis At 2:20PM on December 9,1986, while operating at full power, the Main Steam Isolation Valve on Unit 2 Steam Generator C spuriously closed; the reactor tripped due to low low level indication on the SG as a result of compression of the voids. A 2 x 3 foot section of the 18 inch diameter section piping to the Main Feedwater Pump A, between the block valve in the suction line and the 24 inch diameter header serving both pumps, ruptured and came to rest in an overhead cable tray.

Eight contractor personnel, working on an unrelated activity in an area near the break, were injured by hot water (approximately 400 F) and water flashing to steam. There have been two fatalities and four workers remained hospitalized (current through December 12) in serious to critical condition. The break occurred on the secondary side; as a result there was little radiological consequence.

The licensee stepped through an Unusual Event at 2:30PM and escalated

,N to an alert at 2:40PM. During the recovery effort, the operators and

-u the plant performed as expected. The Agency was placed in standby and a site team was dispatched from Region II; this contingent and supplemental technical specialists were reconstituted as an Augmented Inspection Team on December 10, 1986.

Preliminary information indicates that extensive erosion / corrosion

\ inside the suction piping at the elbow was a major contributor; other areas included in the AIT investigation include the feedwater check valves, erosion of other piping and other event related conditions.

As a consequence of the severe pipe thinning experienced, the licensee commenced a precautionary shutdown of Unit 1 for evaluation of conditions; similar conditions were found.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending December 12, 1986 Plant Risk Status Information Management System (PRISIM)

Risk Analysis Branch personnel and consultants briefed the resident inspectors and regional personnel at the Region II offices in Atlanta (12/4/86) on RES's PRA based inspection system PRISIM (Plant Risk Status Information Management Systen). The briefing consisted of a presentation of the PRISIM program objectives, status and future plans involving Region II participation in the SURRY PRISIM system. Considerable discussion developed during the actual demonstration of the PRISIM system presently under test and evaluation at the ANO-1 Unit 2 plant, where its trial use has already resulted in enhancing the resident inspector's effectiveness. PRISIM allows the resident inspector to evaluate the increase in plant risk brought about by the removal from service of individual or multiple plant components where failures of these components were evaluated in PRAs.

" Revitalizing Nuclear Safety Research" Report Advance copies have been received of the report " Revitalizing Nuclear Safety Research," by the Committee on Nuclear Safety Research of the National Research Council; these have been distributed to the EDO and the Commission.

Full copies, with the appendices, will be available about December 22. The report contains numerous recommendations regarding: Who should pay for, who should carry out, and who should set the set agenda for nuclear safety research?; Program areas in nuclear safety research that require more attention or a different focus; and, solutions for management problems affecting the research program.

Presidential Guidance to Federal Agencies on Occupational Radiation Exposures NRC staff personnel were notified on December 10, 1986, that the EPA Administrator has transmitted to the President for his signature new guidance to Federal agencies on occupational radiation exposures. The guidance, which would replace that issued by President Eisenhower in 1960, is based on current recommendations of the International Ccmmission on Radiological Protection (ICRP Reports 26 and 30). An interagency committee organized and chaired by the EPA developed the guidance; OMB, OSTP, the NRC and all other affected Federal agencies concurren in this guidance. If the t

2 President signs the guidance, as expected, Federal and State agencies will be expected to modify their radiation protection regulations and procedures accordingly. The NRC staff developed a proposed major revision of related NRC regulations (10 CFR Part 20) in parallel with the guidance development effort. The proposed revision, which was published for public comment on January 9, 1986 (Federal Register, Vol. 51, No. 6, p. 1092), is intended to implement the new Federal guidance and is therefore generally consistent with its provisions. The comment period for both the rule and the associated backfit analysis closed October 31, 1986. Over 800 comment l 1etters have been received, and a preliminary analysis of these comments is being conducted to help determine whether and how to continue this rulemaking. While the proposed revision to Part 20 involves changes that are specific to NRC concerns not covered by the new Federal guidance, many of.the proposed changes will have to be made if compliance with the guidance is to be achieved.

[ Review of Source Term Research In response to the Commission's Staff Requirements request of July 19, 1986, RES has developed a research plan to resolve and review each of the problem areas of technical uncertainty identified in NUREG-0956.

The research plan was transmitted to ED0 on December 8. The review of the research plans and areas of uncertainty is being conducted for RES by BNL and includes the use of independent experts as suggested by the Commission. BNL is proceeding with the preparation for the review and they will commence during the week of January 5,1987.

On December 10, 1986 the OGC (Fitzgerald) sent a letter to V. Stello concluding that the proposed expert review of these research plans is not barred by either the letter or the spirit of FACA. The letter also will have precedential value for future review activities of this type.

These studies provide cost estimates that have wide application to a large number of regulatory impact analyses ongoing at the NRC. The first, "The Identification and Estimation of the Cost of Required Procedural Changes at Nuclear Power Plants," estimates industry costs associated with changes in written procedures, training, training manuals, recordkeeping, and reporting requirements. l OFFICE FOR ANALYSIS AND EVALUATION OF OPERATIONAL DATA ITEMS OF INTEREST Week Ending December 12, 1986

1. AE00 issued a Case Study report, AE0D/C603 entitled "A Review of Motor-0perated Valve Performance." The study was performed in response to Action Item 6(1) of the actions directed by the ED0 to respond to the NRC staff investigation of the June 9,1985 event at Davis-Besse. The report brings together previous NRC studies and other related documents (i.e., AEOD reports and IE bulletins, circulars, and information notices), reviews approximately 1200 valve operator events (SCSS and NPRDS) from 1981 to mid-1985, and incorporates new data from an NRC valve testing program that utilized signature tracing techniques on valve assemblies in operating plants.

The report concludes that current methods and procedures at many operating plants are not adeouate to assure that valves will operate when needed. Furthermore, the deficiencies would generally not be detected by existing plant procedures intended to assure operability, such as surveillance testing (plant technical specifications and ASME Code,Section XI inservice testing) or plant operator observations. Thus, assurance of valve operability appears to be strongly dependent upon diagnostic capability to assess and evaluate failures to operate so that root causes of failure--including switch setpoint--are correctly determined and proper changes implemented. Further, the issue of valve assembly performance and reliability is a complex subject that involves several technical disciplines.

AE0D believes that a concerted, high priority licensee effort is needed to develop and implement improved guidance, procedures, and/or equipment to address all aspects of safety-related motor-operated valve assembly operability. This effort should cover the issues addressed by the four recommendations in the report. The ED0 has requested that NUMARC undertake the appropriate initiatives to remedy the motor-operated valve performance and reliability problems.

2. AE0D issued a Case Study report, AE0D/C604 entitled " Effects of Ambient Temperature on Electronic Components in Safety-Pelated Instrumentation and Control Systems." The report documents the review and evaluation of four events at nuclear plants involving failures of solid state electronic components in safety-related I&C systems due to overheating. These events occurred at McGuire 1, Davis-Besse, Summer 1 and Palo Verde 1. Overheating of electronic components raised two concerns: (1) decreased reliability i of electronic equipment due to increased failure rate of electronic components, and (2) the potential for common cause failure of redundant safety-related instruments due to extended loss of normal cooling air flow to the cabinets in which the instruments are located. Based on the findings and conclusions of the study, recommendations which address these two concerns are provided in the report. The recommendations address:

with loss of cooling to instrument cabinets; (2) the need to monitor actual conditions (specifically, temperature) 11 instrument cabinets; (3) the need for plant technical specification requirements governing the operability of control room cooling and ventilation systems which reflect the actual temperature in the instrument cabinets; and (4) the need for specific considerations of this issue in the plant specific evaluation of the station blackout issue (USI A-44). This case study has been forwarded to NRR for appropriate action or for further follow-up by industry.
