The following information is submitted in response to Supplemental Inspection Report Nos. 50-321/86-08-01 and 50-366/86-08-01, dated June 16, 1986.

The report concerns the inspection performed by Messrs.

G. A. Schnebli and N. K. Poertner of your office from February 24 through March 3,1986.

f VIOLATION:

"10 CFR 50, Appendix B, Criterion XVI states:

The identification of the significant condition adverse to quality, the cause of the condition, and the corrective action taken shall be documented and reported to appropriate levels of management.

10 CFR 50, Appendix B, Criterion Y states:

Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, I

of a type appropriate to the circumstances and shall be accomplished in i

accordance with these instructions, procedures, or drawings.

Hatch procedure 10AC-MGR04-0, Deficiency Control System Step 8.2 states:

Significant conditions adverse to quality will normally require initiation of a Deficiency Report (DR).

Contrary to the above, on January 3,1986, when it was determined that the replacement wedge for valve lEll-F004A did not fit the original stem, the stem was ground down to make the replacement wedge fit.

This modification of the original stem was accomplished on the verbal authorization of the maintenance supervisor only.

No deficiency report was initiated to document that the replacement wedge did not correspond to the as-found condition of the valve or that modification of the original stem had been accomplished by disregarding the administrative controls and procedural requirements of the Hatch maintenance program.

This is a Severity Level IV violation (Supplement I)."

l RESPONSE TO VIOLATION:

Admission or denial of alleged violation: The violation occurred.

Reason for violation:

Plant personnel failed to recognize this anomaly as a "significant condition adverse to quality" per Deficiency Control Procedure 10AC-MGR04-0S.

This failure was due to a misinterpretation by plant personnel of the requirements stated in 10AC-MGR04-05 for identification of an adverse condition.

Corrective steps which were taken and the results achieved:

Deficiency Report 1-86-295 was issued to document this anomaly.

Management has emphasized to plant personnel the importance of generating deficiency reports, when applicable, per Management Bulletin MM-MGR-002-0386.

These steps have created a heightened awareness by plant personnel of the need to document known deficiencies.

Corrective steps which will be taken to prevent further violations:

The corrective actions already taken are sufficient to prevent recurrence.

Date when full compliance was achieved:

Full compliance was achieved on March 1,1986, when Deficiency Report 1-86-295 was issued.
