SUIMECT: EIGirrH MAP MEETING IIELD ON JANUARY 13,199k The eighth Millstone Assessment Panel (MAP) meeting was held at the Millstone site resident office from 8:45 a.m. 10:15 a.m. The attendees at the meeting were:

The meeting started with a discussion bypaul Swetland of the status of each of the Millstone Units. Currently, Unit 1 is operating at 100% power. NU is pursuing with the NRC a TS change and temporary waiver to bypass the signal from the main steam line radiation monitors for two hours to avoid the potential of a reactor trip and MSIV isolation while putting a condensate demineralizer in service. In the past, this evolution resulted in the admission of a slug of air into the reactor that resulted in radioactive Nitrogen 16 actuating the main steam line radiation monitors, causing the closure of the main steam isolation valves and a reactor trip. On December 29,1992, spikes were also observed on the MSLRM while placing a condennte demineralizer in service, although no trip setpoints were reached in this case. Unit 2 is operating at 20% power after restarting from the Cycle 11/12 reh.eling and steam generator L replacement outage. An NRC Operational Safety Team inspection (OSTI) has been observing tha restart preparations since December 28,1992; the Gndings of the team are discussed later in l

tLac minutes. Unit 3 is at 100% power after returning a motor driven feedwater pump to service that had previously lir x! power operations. T inadvertent over611ing of the oil lubrication reservoir on the , ,, caused its failure in late December.

Rich Barkley p~.sented the MAP with the preliminary findings of the Operational Safety Team Inspection (OSTI) conducted during the period from December 28,1992 - January 14, 1993.

The charter of the team inspection as well as the checklist of restart activities that the MAP requested that the team review is provided as Enclosure 1. Overall, the team found that NNECO ,

satisfactorily completed the refteling and steam generator outage and retumed Unit 2 to se.rvice in a safe and conservative manner. All equipment out of service for maintenance and repairs was returned to service prior to the time required by the Technical Specifications.

The team did note that NNECO did not complete a number of the procedural updates required in light of the plant design changes completed during the outage prior to changing plant modes.

Reviews by the team and discussions with NNECO indicated that the procedure changes that remained to be completM were, for the most paro not very safety significant or were known to  ;

the operators via other means (e.g. training on design changes, night orders) and the procedure .

changes were in the process of being made. Nevertheless, the delays in implementing these.

procedures were inconsistent with administrative procedural requirements, indicative of poor planning and were not consistent with NNECO management's stated goal of enforcing strict procedure compliance, in addition, the team has several concerns with NNECO engineering -

regarding the approval process for design changes and field change requests to modification packages. ,

The team is currently completing their activities, will meet among themselves next week to finalire their findings and will brief NRC management of their conclusions late next week. The final team exit will be conducted with Unit 2 managemen; either by phone or in peron during the week of January 25,1903.

The MAP accepted the recommended NRC actions in response to the comments received at the November 5,1992, public meeting on the PEP. All of the recommended actions were referred to the appropriate parties; the completion of these actions is expected in the near future.

Pollowing the MAP meeting, the MAP held a public meeting with Northeast Utilities senior management regarding their progress on the PEP. De results of that meeting will be provided by letter to NU in the near future and sent to those on distribution for these meeting miautes.

ne MAP discussed the preparations for the meeting with NU and the Commission tentatively scheduled for February 9,1993. John Stolz drafted a tentative series of slides for the NRC staff to talk from after discussions with DRP. John will coordinate the NRC staffs presentation with NU's presentation to ensure that the two do not overlap.

Comments on the PEP Review Team Renort/ Status of the SER o i the PEP; The draft report of the PtiP Review Teani was provided to the MAP members for their review and comment at the last meeting. That report will largely form the basis for the draft SER.

The MAP will begin drafting the Safety Evaluation Report on the PEP in the near future. The SER should be in a draft stage at the time of the Commissien meeting and completed shortly thereafter provided no significant concerns with the PEP ve ification and validation program remain (this portion of the PEP is in its infancy and underwent significant revisions in the last several months).
