s MILLSTONE ASSESSMENT PANEL MEETING MINUTES MAY 4,1995 ATTENDEES: P. Mc Kee J. Jacobson D. Beaulieu J. Andersen R. Arrighi J. Trapp G. Vissing P. Swetland R. Cooper J. Durr A Millstone Assessment Panel meeting was held on May 4,1995, to discuss the items listed in the agenda, Attachment 1. The meeting opened with a briefing of plant status by the senior resident inspector, P. Swetland and his staff.

Unit 1 is operating at full power and is expected to remain at power until the shutdown in October 1995. Unit 3 ii in refueling, Mode 0, and work is progressir.g on schedule, without inc. % t. Unit 2 is in Mode 6, the loss of normal power test is scheduled for this weekend; the refueling went well.

Themilestoneschedule(Attachment 2)wasreviewedandadiscussionofthe float times of the immediate milestones ensued. The likelihood of the RATI start date slipping was discussed, and it was decided that any slippage in the licensee's schedule of one or two weeks would not postpone the inspection.

The RATI team leader, J. Jacobson, briefed the MAP on the aspects of the RATI stating that the entrance interview would be held on May 8, 1995 at 1:00 p.m.

The team will receive a briefing by the licensee on the Improving Station Performance" ilan and restart issues. The team intends to work the week of May 22 througi 26, taking the holiday weekend off and returning on May 30.

NRR is working two license amendments, one dealing with the status of the charging pump when changing from mode 5 to 4. This is an exigent technical s)ecification amendment and should be issued on May 18. The second deals with t1e feedwater isolation and is not needed until the plant is approaching power. NRR is processing this change, also.

The MAP discussed what issues should be addressed by the licensee at the restart meeting. A proposed list was presented (Attachment 3) and discussed.

The MAP tentatively accepted the list recognizing that it may change as the inspection progressed.

A question arose during the meeting on the status of MC 0350 items and which ones were being closed by the RATI and the resident inspectors. L. Nicholson was tasked.(in absentia) with resolving the question on his return from training. An updated version of MC 0350 would be developed and placed on the docket. Additionally, the responsibility of the RATI to close open items was also discussed. The decision was made that where open items interface with the course of the inspection, they would be closed. However, there would be no

.pecial effort on the part of the inspection team to close items beyond the scope of the inspection.

The panel discussed the best option for the sequence of the RATI exit meeting, the licensee restart meeting and the NRC acceptan:e letter. It was decided that J. Durr would develop the various models and present them to the panel.

After comments were incorporated, the models and any recommendations would be

q presented to T. Martin for approval.

The MAP reviewed the licensee's Startup Assessment Team preliminary findings.

The RATI team leader will review the findings and reconcile them with the RATI scope of inspection.

Various issues presented by the resident inspectors were discussed and resolved. J. Durr was tasked to verify the status of the team site access letters.

L. Nicholson Verify the MC 0350 items being closed by the RATI and the resident inspectors.

Update the MC 0350 checklists and prepare for issuing on the docket.

NORKIN READINESS ASSESSMENT TEAM HEMBERS The meeting was called for the Millstone Assessment Panel to hear the findings of the Readiness Assessment Team. The first order of business was to define what constituted a " restart issue." The MAP concluded that a restart issue was one that was a deficiency having a regulatory basis that would make the operation of the plant unsafe. Other items would be considered on a case by case basis for recommending it as a restart issue.

The meeting was turned over to the RATI team members to discuss the details of their findings and for the MAP to determine if the individual items constituted a restart issue. The items discussed are contained in Attachment 1. The following items were assessed as restart issues by the MAP:

  • An electrical tagging error occurred during the maintenance of a motor o>erated valve. The o)erators depend on the 1st line supervisor to ensure t1e tagg ng boundary aecause they are not technically qualified to read the deta led electrical drawings. The licensee is currently using the

' electrical technicians and the 1st line supervisor for ensuring the accurac/ of electrical tagouts. The licensee performed a root cause i C analysis and needs to impicment the corrective actions in a timely manner.

  • MAP should assess the need to control the punch list items.

The MAP needs to assess the SAT findings and determine if they are restart j issues. Specifically, the operational configuration control issues.

  • The MAP determined that because the AITTS is part of the corrective action program (provides tracking and closure of corrective actions) the licensee should ensure key people are trained and the deficiencies in the program are corrected in a timely manner. Need a commitment on the foregoing and a schedule for completion.

The MAY discussed the need to establish NRC ex)ectations on the controls over items on the Unit No. 2 Restart Punch list. T1ey concluded that the licensee should provide the NRC with a current status of open and closed items, assurance that any items deleted or the scope changed would be approved by the Plant Operations Review Committee, and inform the resident inspectors of any of the foregoing changes. The NRC resident inspectors will assess any of the Olv

deletions or changes to determine if the alterations are acceptable.

The format and content of the MC 0350 letter was discussed. L. Nicholson ,

presented a proposed draft for comment. The MAP members will provide comments to L. Nicholson no later than June 12, 1995. The proposal will be provided to the regional and headquarters management for further guidance.

The MAP decided to provide the licensee with the restart issues by the following Monday to allow time for the licensee to develop adequate responses, where possible, before the Restart Meeting on June 14, 1995.

The MAP discussed the current milestones and schedules-to establish priority activities (Attachment 2 and 3). The MAP will-reconvene on June 13, 1995, at 8:30 a.m. to discuss the licensee's Startup Assessment Team Report findings to determine if we should endorse any of the findings as restart issues.

Additional agenda items include a briefing by DRS of the ttatus of the Emergency Operating Procedure inspections, gener:1 ins ection attributes and information necessary to close the issue. The MAP wil meet again on June 1995 to consider the E0P inspection results, the status of the ESAS/ ,. ,

sequencer problems, and closeout status of MC 0350 issues, tr '.. y -

The MAP discussed what the content of the June 14, 1995 Restart Meeting with the licensee should be. They concluded, as a mihimum, the meeting should address the following:

1. The licensee should address the May 25, 1995, letter and the contained recommendations of the SAT, NSAB,and the vice president.
2. Generally, what occurrences caused the extended shutdown , what has been done to correct the problems, and what performance standards and acceptance criteria currently indicate.
3. The licensee should discuss their current actions for the NRC Readiness Assessment Team Inspection findings where appropriate, recognizing that some issues were recently identified.
4. The licensee should discuss the status of the restart punch list items and the Startup Plan.

The foregoing information regarding the controls for the punchlist and the meeting content was discussed with the senior vice president for the Millstone station.

The Millstone Assessment Panel met to discuss the status of the emergency operating procedure uxiate and the associated operator performance issues. G.

Meyer presenteJ the D' vision of Reactor Safety's current plans to reinspect the licensees training of operators and completion of the E0P update. He noted that licensee progress has been slow and that the operators have not been doing well in their training-scenarios; however, the licensee is identifying the problems and taking the appropriate actions. He stated that the NRC reinspection may occur as early as next week. At least 3 crews have had failures which appear to be attributable to a loss of focus. Additional issues include the lack of validation of support procedures and availability of equipment. There will be a 10:30 call with the utility to discuss the latest status and snsure the issues are clearly identified and the licensee is aware of our concerns.

The next order of business discussed was the licensee's Startup Assessment Team findings and whether they should be considered as restart items by the MAP. The reference document was the licensee's letter, dated May 25, 1995, that presented a synopsis of the team's findings and eight recommendations. The MAP determined that the findings should be separated into three categories. The first category being issues that are significant enough to be classified as " restart items."

The second category is items that warrant an endorsement from the NRC as significant. and lastly, items that, although a concern, and will provide a betterment of th,e plant do The items were categorized as follows:not warrant special emphasis by the NRC.

1. Manaamment and Oraantration Generally deals with the communication of management expectations through the startup plan. The MAP

-categorized this item as warranting an endorsement.

2. Doerations and Trainina This items deals with the licensee's efforts to upgrade the emergency operating procedures and train the operators in them. This is considered a restart issue and has been connunicated to the Itcensee.
3. The SAT noted the- liefted number of licensed operators and recommended the itcensee ensure there is adequate staff to meet all operational demands. This issue was identified by the RATI as an

-observation and does not warrant further emphasis.

4. Item 4 relates to the weaknesses in the tagout procedures and the electrical breaker event that occurred. recently. This was

. identified by the RATI as a restart issue and will be carried by the

- MAP accordingly. ,

5. Enaineerina and Technical Suncort The SAT identified the need to ensure the system readiness reviews are thorough and complete for each operating mode. The RATI identified a weakness in this area relating to the AITTS. The MAP will carry this item as a restart item.
6. Item 6 deals with the cumulative affect of operator burdens, by pass jumpers, and control room deficiencies. The MAP concluded that this is being controlled through other mechanisms and has determined this does not warrant additional emphasis.
7. This item deals with the outstanding plant design changes and engineering work request that may pose a risk if deferred beyond the restart. The MAP endorses this items as warranting additional emphasis.
8. falyfetive Action and Self Assesscent Item 8 relates to configuration control and effective root cause analysis of errors in this area. This is one of the major issues that prompted the eFlended shutdown of the unit. This is considered a restart issue.

The panel discussed the need for moving the next meeting from June 26 to June 21 to provide sufficient time to develop the MC 0350 letter. Specifically what items should be discussed in 'ihe letter and what supports the closecut of the line items in MC 0350. The next meeting will be held on June 21, 1995.
