This refers to the routine safety inspection conducted by Mr. A. A. Varela

of this office on May 5-9, 1986, at GPU Engineering Office, Parsippany, New

Jersey and at the Oyster Creek plant of activities authurized by NRC License

No. OPR-16 and to the discussions of our findings held by Mr. Varela with

Mr. R. F. Wilson at the conclusion of the inspection.

Areas examined during this inspection are described in the NRC Region I

1

Inspection Report which is enclosed with this letter. Within these areas, the

}

inspection consisted of selective examinations of procedures and representative

j

records, interviews with personnel, and observations by the inspector.

i

Based on the results of this inspection, it appears that two of your activities

were not conducted in full compliance with NRC requirements, as set forth in

j

the Notice of Violation, enclosed herewith as Appendix A.

These violations

i

have been categorized by severity level in accordance with the revised NRC

i

Enforcement Policy (10 CFR 2, Appendix C) published in the Federal Register

e

Notice (49 FR 8583) dated March 8, 1984. You are required to respond to this

i

i

letter and in preparing your response, you should follow the instructions in

{

Appendix A.

Another activity appears to be a deviation in that the evaluations performed

!

regarding mortar strength were based on assumptions, and specifications, not on

i

construction records or tests.

This deviation is described in Appendix B.

You

are requested to also respond to the deviation following the instructions pro-

vided in Appendix 8.

Based on the results of this inspection, we were not able to verify that you

had fulfilled the commitments made in your responses to IE Bulletin 80-11,

Masonry Wall Design. The lack of documentation regarding the procedures and

i

i

criteria used in surveys performed to identify those walls which would be

i

subjected to detailed analysis, and, regarding your verification of mortar

f

strength led us to question the adequacy of your actions. Consequently, in

1

your response to the attached Notices, you should include a discussion of why

{

your proposed masonry wall reverification program will adequately address the

i

concerns identified in the Bulletin. Also, you should address the schedule for

i

completion of the reverification efforts.
