1.0 INTRODUCTION

The purpose of this supplemental summary report is to report on those Detailed Control Room Design Review (DCRDR) activities to be completed prior to exceeding five percent of thermal rated power as committed to in various CEI/NRR correspondence and as required by our license (NPF-45, Attachment 3). Ihe following summarizes those requirements:

1. Control Room Survey ~ Provide for NRC review, the results of a human factors survey of. communications equipment and an interim sound survey in the control. room and remote shutdown facilities.
2. Augmented Verification - Provide for NRC review, the results of the augmented process for verifying dhat selected improvements provide {

dhe necessary corrections and do not introduce new Human Engineering  ;

Deficiencies (HEDs). Include the assessment of concerns identified  ;

in Appendix D of the Technical Evaluation Report and selection and verification of design improvements which resolve those concerns.

Provide commitments and implementation schedules for correction of HEDs identified in the above results; or provide justifications for determining not to correct or to only partially correct dhem.

3. Implementation - Complete implementation of design changes for HEDs 93, 500, 517, 519, 523, and 527.

2.0 CONTROL ROOM SURVEY Objective: l Review and assess the adequacy of the Control Room and Remote Shutdown Panel communication systems and noise environment to identify potential modifications of the operator / control room interface which will reduce the potential for human error.

Methodology:

De BWROG methodology for performing a control room survey was utilized. Details of the BWROG development and methodology were included in Attachment A to the January 10, 1985 DCRDR Summary Esport.

Procedure:

I h e control room survey was conducted by the survey team using the BWROG checklisto and supplemental checklists as shown in Attachments B and C of the January 10, 1986 DCRDR Summary Report. Survey and supplemental checklist section F1 and SF1 were performed for the communication survey, and items F2.5, F2.6, F7.1, and F7.2 for the interim noise survey.

Review Team:

De control room survey team was comprised of the PNPP Human Factors Unit Lead Engineer and a Human Factor Specialist.

Documentation:

De results of the surveys are documented on the control room survey checklists. Assessment of the HEDs and selection of design improvements was performed in accordance with section 2.4 of the January 10, 1985 DCRDR Summary Report. Seven HEDs (600 through 606) were identified, and corrections and implementation dates proposed.

Copies of the HED sheets are provided in Attachment A.

4.0 ASSESSMENT OF TER APPENDIX D RECOMMENDITIONS Several recommendations resulted from the NRC Staff's evaluation of the PNPP DCRDR and are documented in Appendix D of Appendix P to the PNPP SSER 7. These recommendations were not intended as additional requirements, but were intended to encourage the fullest possible benefit from the DCRDR. The PNPP Human Factors Team has assessed each recommendation for possible control room improvement. A summary of this review is provided in Attachment C. Justification is provided for those recommendations that will not be incorporated. HEDs are referenced for those recommendations that will be incorporated with the exception of comment 20. That review will be performed and tracked as part of the PNPP ongoing Human Factors Program.

5.0 IMFLEMENTATION .

The following is the status of the remaining open HEDs and their proposed implementation schedules. HED sheets have been provided in Attachment D.

6.0 CONCLUSION

A sumary of the activities remaining to complete the PNPP DCRDR is provided below:

1. Control Room Survey - The final sound survey will be completed prior to startup following the first refueling outage. HEDs resulting from these surveys will be provided with proposed fixes and an implementation schedule prior to startup from the outage.
2. Verification - The augmented verification for those HEDs to be completed prior to exceeding 5% power will be conducted prior to startup following dhe first refueling outage. Verification of the remaining HED related modifications will be completed no later Chan startup following dhe second refueling outage.
3. Implementation - Section 5.0 of Chis report provides the implementation schedule for the remaining HED improvements. A followup letter confirming completion of those HEDs corrections required prior to exceeding 5% rated power will be submitted to the NRC prior to exceeding 5% power.

HED REPORT SHEET HED-601 HED DESCRIPTION: F7.1 - Noise levels at the EHC instrument panel (P821) is 9 dB(A) cbove recommended maximum noise levels of 65 dB(A). This panel is not near an operating crea, however it is close to the unit supervisors workstation and may be causing elevated conversation levels. Noise levels at the unit supervisors work station were just below 65 dB(A) due mostly to people standing at the reception desk processing work orders.

farther away from P821 and the primary operating area and work order processing ,was revised to reduce noise levels at the workstation. After carpeting is installed a final noise survey will be performed to evaluate whether or not improvements must be made to reduce the noise from P821.

1. Red First Priority Alarms

, s. Plant conditions which if not immediately corrected (within 2 minutes) will result in automatic plant shutdown, or will require manual plant shutdown.

2. -Amber-Second Priority Alarms
a. Plant conditions which, if not corrected, may lead to:

Review: ' The PNPP Control Room Standards define amber indicating lights as meaning manual operation or control which is consistent with the yellow manual indicating light. Green is a non-attention getting color which seems appropriate for the green automatic indicating light. Since the "A" and the "M" i

are printed on the indicating light and operators are trained to know what these lights mean on all controllers, we do not

Double ended arrows would add confusion in the referenced portion of the RHR mimic. The additional arrow would have to be placed in the primary injection line showing flow away from the vessel.

15. Comment: Recommend mimic symbols (arrows, pumps, etc.) be included i~n the PNPP HUMAN FACTORS CONTROL ROOM STANDARDS.

Review: Mimic symbols have been included in the PNPP Human Factors Standards.

16. Comment: Recommend that computer keys be designed so that dirt does not impair the readability of the information on them.

Review: The process computer keys were cleaned at fuel load. New keys will be provided so that dirt does not impair readibility (see HED-182).

17. Comment: Recommend enhancement aid on Division 3 WIDE RANGE AND FUEL ZONE REACTOR WATER LEVEL INDICATING RECORDER on Panel 601-17B to increase the operator's ability to acquire necessary information while operating in the HPCS area.

Review: Color banding will be provided on B21-R615 recorder to fulfill the first refuel requirements of HED-84.

18. Comment: Recommend review of annunciator readability from annunciator control stations, especially on vertical panels.

Review: All panels with expection of P800 have annunciators that are [

readable from the control stations. P800 has one set of annunciators that are difficult to read from the annunciator control station. This has not caused any problems with responding to alarms. The operator will silence the alarm and then read it before he acknowledges it.

19. Comment: Recommend addition of " scribble plates" so the operator can denote what parameters have been selected for display on the three process computer digital displays on Panel 680.

R eview: Scribble plates will be provided near the three digital displays to indicate selected parameters (see HED-608).

20. Comment: Recommend review of all the different uses for amber and reduction of that number.

Review: Amber lights will be reviewed for consistent application as part of the ongoing Human Factors program. Changes will be f made as necessary.

21. Comment: Recommend color banding of meters associated with "HI/LO" annunciators to aid quick recognition of whether parameter is HI or LO.

Review: The majority of the meters supporting the HI/LO annunciators have been color banded for quick recognition of whether the paramenter is high or low. The following meters require banding and will be completed prior to first refuel as stated in HED-84.

Review: The annunciator wording has been changed to " FUEL XFER PUMPS TRIP FUEL DAY TANK LEVEL HI/LO" to bring into compliance with the fuel day tank level display.

23. Comment: Recommend that consistency between control room, DCRDR Summary Report, and drawing be confirmed:

24. Comment: Recommend that PNPP HUMAN FACTORS CONTROL ROOM STANDARDS be revised to include PSID if that abbreviation is used in the Conrol room.

Review: PSID has been added to the standards.

25. Comment: Recommend scheduled check (e.g., daily) of printer paper and ribbons, recorder paper and ink, etc. to assure that I&C  !

technicians can be notified of service needs in a timely manner.

Review: IAP-0507 procedures covers these daily maintenance items.

Comment: Recommend that an administrative procedure addressing coordination between the control room and computer room be developed (specifically addressing timely provision to the control room of any output directed to the backup printer in the computer room when the control room printer is out-of-service)

Review: During the verification it was recommended that an alarm be i added to the process computer alarm CRT to inform the operator that the backup printer is in use (HED-195). If the operator needs a printout he can send the I&C Tech down to the computer room to retrieve it. No administrative procedures are recommended.

27. Comment: Recommend that resets and indicators in the control room be reviewed for consistency of operator interface (e.g.,

833A-S110 is not consistent with other RCIRC system reset indication). Further recommend that consistent approach be developed, documented in the PNPP HUMAN FACORS CONTROL ROOM STANDARDS, and implemented in the control room. l Review: PNPP Human Factors Standards will be revised to indicate that consistency should be maintained between related controls and indicators. Two white indicating lights will be added to switch B33A-3110 to bring it into compliance with the Standard (HED-612). -

HUMAN FACTORS REVIEW: An integrated detailed review of Control Room annunciator windows has been conducted. Alarm prioritization, location, alarm logic, readability, terminology and adherence to standard abbreviations were reviewed to aid immediate operator response.

Annunciator windows throughout the control room are grouped by panel section. Annunciator trip alarms that were rearranged into functional groups were the following: DG initiation and trip; ECCS initiation; RCIC initiation, turbine trip and isolation; SPMU initiation; Main Steam Line isolation; NS4 isolation; SRV; ADS; and RPS. This arrangement will improve the man-machine interface with those systems requiring prompt attention and immediate operator action. Annunciator priority has been established by a consistently cpplied color code. This annunciator improvement has been introduced at the simulator.

The Task Force did determine that keylocked switches were not consistently used in the control room. -Inappropriate keylocked switches such as those used for the Inop and Bypass, and the SRV controls will be replaced. The inadvertent bumping and subsequent actuation of a keylocked SRV at Grand Gulf on 10/13/84 lead the team to assess the use of normal key-in-place keylocked switches on vertical panels as providing unsatisfactory safety and operability. The new design intent for keylocked switches used for bypass, restraint and test functions is that during power operation all keys should be removed. Inappropriately used'keylocked switches will be replaced.
