3.0 RESPONSIBILITIES 3.1 lhe Manager, Plant Services Quality is responsible for:
a.
Qualification and certification of inspection and testing personnel.
b.
Administration and interpretation of this procedure, c.
Main taining ef fective control and integrity of tests and examinations.
d.
Appointment of a Level III Examiner to administer any/or all of the duties described in 3.1 (a,
b, c) above as determined necessary.
3.2 The Quality Programs Manager shall be responsible for the control and revision of this procedure.
3.3 The Department Level 111 Examiner is responsible for maintenance of Personnel Qualification and Certification Records and for assurance that re-evaluations are performed as required.

Field Quality Procedare 4.0 GENERAL 4.1 This procedure applies to Qualification and Certification of GE-UPSD Personnel.
When Subcontractor Personnel are
- utilized, the Subcontractors Qualification and Certification Program shall be approved by the Territory Manager, Quality Assurance.
4.2 Prior to performance of inspection, examination or testing activities on a proj ec t, assigned personnel will be indoctrinated as to the technical objectives of the project, the Codes and Standards that are to be used, and the Quality Assurance / Quality Control elements that are to be employed.
4.3 NDSD Personnel will be trained in the proper use of unique project procedure ( s ) for which they will be required to perform inspection, examination, and te s ting.
Records of training to these procedures will be maintained in the site training file.
On-the-job participation shall also be included in the program, with emphasis on first-hand experience gained through actual performance of inspections, examinations, and tests.
4.4 NPSD Personnel who are assigned the responsibility and authority to perform functions covered by this procedure shall have, as a minimun, g
the level of capability shown in Table I.
When a single inspection V
or test requires impl ementation by a team or group, personnel not meeting the requirements of this procedure may be used in data taking assignments provided they have been administered sufficient training to ensure an acceptable level of competence in the performance of their activities and are supervised by a
qualified individual participating in the inspection, examination or test.
4.5 Inspections and tests that require qualification and certification in accordance with this procedure may include, but are not limited to; mechanical / dimensional inspections or tests, electrical inspections or tests, weldment inspection or tests, operational startup or tests.
4.6 NPSD personnel presently certified in accordance with prior approved procedures need not be recertified to this procedure untti t;.e i r present certification expires unless otherwise deemed necessary by the Manager, Plant Services Quality.
4.7 Individuals holding current certification from other General Electric components or General Electric subsidiaries may be utilized by the NPSD projects without recertification if the individual s certification program satisfies the requirements of ANSI N4 5. 2.6 -
1978, NQA 1 and this procedure.

-I Field Quality Procedure Revision 01 S.0 REQUIREMENTS FOR CERTIFICATION 5.1 Personnel who perform inspections, examination or tests and veri fy confonnance of work to quality requirements shall be certified.
5.2 Certification shall be based,n education, training and evaluation of I
pe r f onnance.
Personnel shall be certified for three year intervals and recertified as required to maintain certification.
The certi fica ts of qualification shall include the activities the i
individual is qual i fied to perform or
- witness, level of certification, and the date of certification.
5.2.1 Levels of Certification A Level I person shall be capable of a.

performing the inspections, and tests that are required i
to be performed in accordance with documented procedures, instructions and/or industry practices.

individual shall be familiar with the tools and equipnent to be employed and shall have demonstrated 4
proficiency in their use.

A Level !! person shall have all of the b.

Additionally, a Level 11 person shall have demonstrated capabilities in planning inspections, examinations and tests; in setting up tests including preparation and set-up of related equipment, as appropriate; in supervising or maintaining surveillance over the inspections, examinations and tests; in supervising lower level personnel; in reporting inspection, examination and testing results; and in evaluating the validi ty and acceptability of inspection, examination and test res ul ts.
A level 11 individual may also perform as a Level I.
c.
Level I I _I
- A Level III perr.on shall have all of the capabilities of a level Il person for the inspection, examination or test ca te gory.
In addi tion,
the individual shall also be capable of evaluating the adequacy of speci fic programs used to train and test inspection, examination and test personnel whose qualifications are covered by this procedure.

5.2.2 EEJeation and Experience The following is the minimum requirements that must be satisfied for each level of certification.

Hi gh school graduation or GED equivalent of high school diploma and six months of related experience in equivalent inspection, examination or testing activities, or 2.
Completion of college level work leading to an Associate Degree in a related discipline pl us three montns of related experience in equivalent inspection, examination or testing activities, or 3.
Four year college graduation in a related discipline b

5.1.3 Vision Test a.
An exanination shall be administered annually to assure l
natural or corrected near vision acuity is sufficient to read a Jaeger J -l or equivalent (as a minimum in at least one eye) and f ar vision acuity which shall be a Snellen traction of 20/30 or equivalent as a minimum.
The examiaation shall al so include col or perception l
evaluation using an ISHIHARA's test or equivalent.

successful practical demonstration of proficiency in the inspectiens or tests to be performed wil1 be considered sa ti s f actory and shall be noted on the Vision Acui ty Examination Record - attachment 0 (Typical).

The Manager, Plant Services Quality or his delegate shall document the extent and content of the evaluations.
Evaluation of qualification will be documented on an "Education and Employment Record" Form
( Attachment A - typical).
6.2 The capability of a candidate for certification shall be initially determined by this evaluation of the candidates education, experience and training and either oral or written tests or capability demone tra tion. (Tests shall be employed to certify a candidate in areas where suf ficient experience is not documented but capability can be demonstrated.)
7.0 CERTIFICATION OF OVALIFICATION 7.1 Af ter deterrining the candidate's level of capability, the candidate certification process will proceed as follows:
7.1.1 Certification checklists will be utilized whenever a written examination is not used to evaluate individuals.
The checklists will be utilized to test / evaluate the individual's knowledge and to assure that the minimu:n areas are covered by the evaluator.
The checklist will al so r
provide written documentation that an individual has been properly evaluated.
7.1.2 Checklists shall be designed to evaluate the individuals basic knowledge of the type of inspection or test.
Checklists will be utilized to evaluate whether the individual can read and interpret procedures, specifications and codes.
7.2 Written Certification of Quali fica tion will be documented on the "Evaluation Sumary of Qualifications" Form - Attachment B (Typical) and on the "Ce r ti fica te of Qual i fica tion" Form - Attachment "C"
( Ty pi cal ).
7.3 The certification record shall include the following:

Any person who has not performed inspection, examination or testing 8.2 activities in his qualified area for a period of one year shall be re-evaluated prior to assignment of work in this area by the Hanager, Plant Services Quali ty or the assigned Level Examiner in accordance with Para. 6.2 of this procedure.
