ALLEGTION RECEIVED BY TERMI II SAFETIAM On August 22, 19E3, Nuclear Inergy Services (NES) employee si the f ollowing allegation in his exit interview to the Terr.i II Fefeteam.

T. lute heads in Airlock and Sican Tunnel - About 24 welds that require UT inspection - Inspection could not be perfoITned as documented.
At least 22 or the 24 have. incomplete fusion.

NES was asked to perform I-ray inspection of jet pumps.
Due to configuration of equipment, an oversize
- film was utilized.
The welds that were the targets of the inspection were satis-f actory, but a number of welds adjacent to the target welds were found to be rejectable. Hollingsworth (Wismer & Becker) fe' requested NES to cut film to eliminate reject welds.
The film and reports have been manipulated to conceal rejectable welds.
A surveillance report was issued on t.his but the number is.
unknown. Wayne Leaverett of PDA was also involved in the above situation.

A number of UT reports purportedly signed by the interviewee are forgeries.
The licensee is currently investigating these allegations.
The NRC inspectors will perform a followup of the licensee's action on this ratter and will doeurtent their findings in.a subsequent inspection report.
