I. BACKGROUND On June 9, 1985, the Davis-Bessa Nuclear Power Station experi-enced a loss-of-main-feedwater transient. A number of equipment i

failures and an operator error occurred which resulted in a temporary loss of all feedwater flow. Feedwater flow was recovered through manual initiation of the startup feedwater pump I and the auxiliary feedwater system. The plant was subsequently i brought to a cold shutdown condition. The number of equipment I

failures which occurred during the June 9th event dictated the l

' need for a thorough investigation by Toledo Edison into the causes of the failures and completion of the corrective actions necessary to prevent recurrence of such events at Davis-Besse.

On September 12, the B&W Owners Group Executive Committee decided on a course of action in response to the Davis-Besse June 9 event to achieve the following objectives.

o Improve the safety and availability of the B&W units through close evaluation and implementation of lessons learned from'the June 9 event.

o Establish a mechanism to ensure strong communication between all of the B&WOG utilities regarding the issues and concerns identified during the review of the Davis-Besse transient.

o Establish an executive level forum to identify, track, and ensure completion of plant specific and generic

! recommendations that result from the review of the Javis-Besse transient.

o Maintain a proactive relationship with the NRC.

To achieve the first three objectives, the B&WOG Executive Committee directed the Chairman of the B&WOG Steering Committee l

to form a special Task Force. The Task Force was to be com-prised of representatives from each of the operating plants with a mission to thoroughly review the results of the investigation by Toledo Edison and the NRC to determine if any of the issues raised are applicable to the other B&W plants.

The fourth objective, " Maintain a proactive relationship with the NRC," is'being implemented as follows:

Overview Consistent with the direction provided by the B&WOG Execu-tive Committee, the B&WOG 1154 Task Force was formed and held its organizational meeting on September 25-26, 1985, in Toledo, Ohio. At this meeting a draft version of the action l plan was developed.

The second meeting of the 1154 Task' Force was held in l St. Petersburg, Florida on October 30 and 31, 1985. The l second draft of the action plan was completed at this time.

The above individuals were charged with the responsibility to develop an overall action plan for the B&WOG to coor-dinate these actions related to the Davis-Besse event within their individual utilities, and to provide the results to other members of the Task Force. They will also participate in the development and implementation of any generic activities that are identified for action and resolution by l

The action plan contained in Appendix A of this report was developed by the 1154 Task Force and is intended to address generic and plant specific hardware issues as well as the management and programmatic concerns that have been raised during the investigation of the Davis-Besse Event.

During the review of the two volumes of the Davis-Besse Course of Action, several items were evaluated by the 1154 l

l Task Force as being specific to Davis-Besse and therefore

~ they have not been specifically included in the B&WOG Action Plan. Much of the material in Volume 1 is dedicated to the formulation of the Davis-Besse Action Plan and includes

descriptive material of-how Toledo Edison intends to' fold l activities already underway into their Course of Action.

Items such as the PEP and SALP programs are integrated in their overall corrective action' plan. These programs are quite plant specific. In Volume 2, the sections describing the Toledo Edison Organization are, of course, not addressed in the B&WOG 1154' Task Force Action Plan. Likewise, the analysis of the failure of specific feedwater valve SP.74 is not included in the B&WOG Action Plan.

All'other B&W plant generic issues and programmatic concerns in-the Davis-Besse Course of Action plan, in Mr. Dirck's

! letter of August 5, 1985, and in Mr. Denton's letter of August 14, 1985 are addressed by the B&WOG 1154 Task Force Action plan.

IV. CONCLUSIONS The B&W Owners Group has concluded that the Action Plan developed by the 1154 Task Force and presented in this report clearly demonstrates a proactive and aggressive approach aimed at enhancing the safety and availability of the B&W operating units.

It is further concluded that the implementation of this action plan will substantially reduce the likelihood that an event similar to the one that occurred at Davis-Besse on June 9, 1985, will not occur at another B&W unit.

Minimum Standard Content for Reports on B&W Owners Group 1154 Task Force Action Plan Items i

The following listing of standard contents is intended to provide a consistent basis upon which the B&W Owners Group members may report on their completion of specific action plan items for the 1154 Task Force. Each action plan-item will be acted on as described in the action plan and reported using this standard contents listing. Then reports on each item will be distributed to each of the other Task Force representatives for information and use. These reports will be compiled, consolidated, and finalized in a Final Report which will contain the recommenda-tions of the 1154 Task Force related to the Action Plan items.

Each report should contain the following information:

1. The Action Plan item being reported.
2. The general process used to conduct the action (i.e., .

walk-through, review of procedures only, audit, etc.)

3. A statement of specifics included in the action (i.e.,

review of all Motor Operated Valves; review of docu-ments X, Y, and Z; one hour of training for all auxiliary operators).

4. Results of Action Plan item.
