Sheet of Commerrial Grade Dedication Package Checklist Instructions

'Ihis checklist is used to document the review of Commercial Grade Dedication (CGD) packages. The chkliet may be used to document the review of more than one CGD package, and shall be completed in accordance with the followmg instructions:

1. 'Ihe Veri 6er shall review the CGD package agamet each attribute listed on Pages 3 and 4 of the checklist. Multiples of Pages 3 and 4 may be used when more than one package is being considered.

2. 'Ibe Verifier shall indicate, for each attribute, whether the attribute is satisfactory (Y), unsatisfactory (N), or not applicable (NA).
3. 'Ibe Verifier shall assign a sequential comment number to isyc,sace indicated on Pages 3 and 4 which require clarification orjustification and shall use the Page 5 comment sheet to provide the comments. Multiple Page 5's may be used. 'Ihe justification shall reference the section of the CGD package reviewed for which the comment is applicable.

Discrepancies shall be processed in accordance with PI-MP3-11.

5. The Verifier shall enter the CGD package number on each sheet, number the sheets sequentially, and sign and date the cover sheet. The sheet numbers shall be sequentially numbered (i.e.,1,2,3, etc.). It is acceptable to add insert pages (i.e., I A, IB. IC, etc.), if needed l
6. 'Ihe Verifier shall obtain the PRO Lead's concurrence on the cover sheet that the Ch. age Review has been adequately l

completed.

7. 'Ihe cover sheet and all applicable checklists and comnwnt forms shall be included in the final project file copy.
