' Inspection 84-34 i

l Mr. Collins, Regional Administrator, USNRC, opened the meeting and had all j participants identify themselves as to name, position and company. Following j some brief discussion on agenda of the meeting, the meeting was turned over to j Mr. Peranich, I&E, Bethesda. Mr. Peranich gave a brief history of the Inquiry i Team which was initiated to investigate the allegations brought forward in a

series of Gambit articles. The investigation centered around Quality Assurance activities during the construction of Waterford 3. The basis of the i inspection was the information contained in LP&L letters of September 29, 1983

and February 20, 1984, to the Director, Office of Inspection and Enforcement

! in response to I&E letter dated August 4, 1983.

) o Adequacy of LP&L QA program; LP&L didn't know whether its QA program i was being implemented 1

Mr. Peranich stated that this is Inspection Report 50-382/84-34 and contains eight (8) unresolved items, contained in Appendix C of the inspection report.

Discussion, for clarification, on the eight unresolved items followed. LP&L felt, in some instances, a clear understanding of the LP&L programs was j lacking on the part of the inspectors. It was decided that a followup

meeting, with fewer participants, would be held to clarify the specifies within the inspection report.  ;

f Mr. Collins spoke on " Philosophy" changes that should be instituted by the i industry. This discussion was primarily on the resolution of " root-cause" and

. how the utilities should pay particular attention to this issue as he is

) instructing the Regional inspectors to evaluate issues closely for root cause.

Mr. Collins stated that the proposed enforcement actions outlined in Appendix i B of Inspection Report 84-34, will be considered jointly with the proposed '

j enforcement actions of other reports presently being evaluated by Region IV.
