Routine, Unannounced, Material Control and Accounting Inspection on October 15-18, 1985 (Combined Report Nos. 50-277/85-37 and 50-278/85-38).

Areas Inspected:

Reconstitution of Unit 3, Reload 4, fuel bundles; organization and operation; measurement and control; shipping and receiving; storage and internal control; inventory; records and reports; and management of material control system for Special Nuclear Material (SNM).

The inspection involved 33 inspector-hours onsite by a regional-based inspector.

Results: The licensee's material control and accounting system was found to be well controlled and applicable procedures were being followed.

30703 - Exit Interview The inspector presented the inspection findings to Mr. L. Rubino at the completion of the inspection at the licensee's corporate offices in Philadelphia, Pennsylvania on October 18, 1985. At the completion of the inspection at Delta, Pennsylvania, on October 17, 1985, an exit interview was not conducted due to the unavailability of licensee representatives who were involved in an Emergency Preparedness drill that was in progress at the facility.

On October 21, 1985, the NRC Senior Resident Inspector conveyed the inspection findings to the plant staff.

At no time during this inspection was written material provided to the licensee by the inspector.

92702 - Licensee Action on Previous Inspection Findings (Closed) Violation (81-25-01): The inspector determined that the licensee has established and was following written procedures that provide for the accountability and physical inventories of all fission chambers located at Units 2 and 3.

(Closed) Violation (81-27-02): The inspector determined that the licensee was maintaining Unit 3 fuel location history sheets on a current basis.

92713 - Independent Inspection Effort The inspector reviewed the detailed procedure prepared by the General Electric Company (GE) and approved by the licensee, for the reconstitution of the fuel bundles containing gadolinia fual rods (Gd 0 ) from Unit 3, 2 3 Reload 4, which had experienced crud induced localized corrosion. No discrepancies were identified.

One hundred and one (101), Reload 4 fuel bundles had been reconstituted by replacing selected Reload 4 Gd 0 rods with selected Gd 0s rods from 2 3

Reload 3 donor bundle.

The reconstitution activity began on August 30, 1985 and was completed on September 17, 1985. The procedure was found to contain the following information: the donor bundle and repair bundle serial numbers; the location numbers of the rods that were to be exchanged within the bundles; the Core Component Transfer Authorization Sheet Nos. (CCTAS) that were prepared to document the movement of_the effected bundles to and from storage; and, the initials of the authorized personnel who witnessed the completion of each step.

The procedure was sufficiently detailed to complete the activity and appeared to have been properly executed.

85102 - Material Control and Accounting i

a.

Organization and Operation The inspector verified that the licensee had maintained written pro-cedures for nuclear material control and accounting and that written statements of responsibility and authority had been established for those positions with responsibility for special nuclear material (SNM). The procedures were included in the licensee's A-44-Fuel and Special Nuclear Material Accounting and Safeguards Directives, Re-vision 7, dated August 18, 1982. The possession and use of special nuclear material were confined to locations and purposes authorized by license, as required by 10 CFR 70.41.

b.

Measurement and Controls The licensee had maintained, and was utilizing,a method of computing the total element and isotopic composition of nuclear material in each fuel bundle. The computation of the total uranium / uranium-235 depletion and plutonium production was performed on an on-line process computer.

The licensee's computer tabulation summary of total uranium and uranium-235 depletion and plutonium production agreed with the totals reported on the DOE /NRC Form-742's submitted between April 1, 1981 and March 31, 1985.

Shipping and Receiving The inspector determined through review that the licensee had main-tained procedures to assure that a71 special nuclear material shipped or received was accurately accounted for.

Shipping arrangements for incoming and outgoing SNM were under the supervision of the Nuclear Engineering Section of the Mechanical Engineering Division.

Preparation and transmittal of Nuclear Material Transaction Reports (DOE /NRC Form-741) were the responsi-bility of the Nuclear Section, Electric Production Departmen.

A review was performed of all DOE /NRC Form-741 generated during the period April 1, 1981 to March 31, 1985 to determine proper signature, timely dispatch, and accuracy of data. No discrepancies were noted, d.

Storage and Internal Control The inspector determined through review that the licensee was main-taining and following a system of written material control and accounting procedures which provided for knowledge of the quantity, identity, and current location of all SNM-bearing materials within the facility.

The licensee was maintaining Item Control Areas (ICA's) that included the reactor core, spent fuel storage pool, and other areas that were appropriate for SNM contained in other than fuel.

Transfers of material were controlled by the reactor engineer and were documented on a Core Component Transfer Authorization Sheet (CCTAS).

Inventory The inspector performed an inventory verification on October 16, 1985, which consisted of a piece count of the assemblies in the spent fuel pool and a comparison of the fuel location history card file to the core loading and spent fuel pool map.

The licensee's last inventory was performed September 23, 1985 for Unit 2, and September 24, 1985 for Unit 3.

The inspector determined by a records review that, after the last outage for Unit 2, the presence of the fuel assemblies located in the reactor core and the spent fuel pool was verified by serial nm.ioer and location, utilizing underwater TV equipment.

Records and Reports The inspector reviewed the licensee's records and source data. All Material Balance Reports (DOE /NRC Form-742) submitted by the licensee during the period April 1, 1981 to March 31, 1985 were reviewed as were all Nuclear Material Transaction Reports (DOE /NRC Form-741).

Total uranium and U-235 fission, transmutation and plutonium pro-duction and decay records were also reviewed. No discrepancies were noted.

Exhibits I through IV of this report summarizes the licensee's nuclear material activities for the period April 1, 1981 to March 31, 1985.

Management of Material Control Systems The inspector determined that the licensee was maintaining a manage-ment system which provided for the revision, implementation and en-forcement of nuclear material control and accounting procedures.

There was an organization structure responsive to the nuclear ma-terial control and accounting requirements, the license and ap-plicable NRC regulation.
