I APPENDIX A NOTICE OF VIOLATION University of Washington Docket No. 50-139 Nuclear Engineering Laboratory Reac or License No. R-73 As a result of the inspection conducted during the period of July 17-19, 1985, and in accordance with NRC Enforcement Policy 10 CFR,- Part 2, Appendix C, the follouing violations were identified.

A.

Technical Specifications, Section 11.(3) states in part, "... Annual evacuation drills for facility personnel shall be conducted to assure that facility personnel are familiar with the emergency plan."

Contrary to the above requirement, an annual evacuation drill was not conducted in 1984.

This is a Severity Level IV Violation (Supplement I).

B.

10 CFR Part 50.54 " Conditions of Licenses" states in part (i-1) that "Ilolders of operating licenses in effect on September 17, 1973, shall implement an operator requalification program which, as a minimum, meets the requirements of Appendix A of Part 55 of this chapter which was submitted for approval by the Atomic Energy Commission."

The licensee's NRC approved operator requalification program of October 9,1974 requires that:

..."In the first quarter of each year the Director will prepare and administer a written examination for all licensed reactor operators and senior reactor operators for the facility."

Contrary to the above requirement; written examinations for the University of Washington's Nuclear Engineering Laboratory reactor operators and senior reactor operators were not administered during the first quarter of 1985.

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions 10 CFR 2.201, the University of Washington is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the resuits achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
