Dear Mr. Keppler:
This letter formally transmits our final report which reviews the subject failure and documents the subsequent investigations which were performed to determine the cause of failure.
No new information is enclosed, i.e.,
all the results of our investigations have been provided to you either formally or during meetings with your Staff.
Although the investigations did bring out areas of potential concern basically, a specific cause for the failures could not be determined.
Therefore, even though no final conclusions can be drawn, this report will form a base for evaluation of any future snubber failures.
Please direct any questions you may have regarding this matter to this office.
Due to the size of the report we are only providing one (1) copy.
