F' wa ~- - A - g. Q,' (1-ex- !.y p i 4 MIDLAND ENFORCEMENT PACKAGE General Comments: G. Klingler 1. QA problems identified relative to the D/G's do not appear to meht the criteria of a Severity II. Suggest that a Severity III be used with appropriate multipliers. 2. NOV, pg 14 item 8.a: QC inspectors were not identifying as nonconformances allof the deficiencies which they identified, a. NRC encourages licensee self-identification and correction of violations and to avoid potential concealment of problems of safety significance. Mitigating factors are alloied for this action. Ineffective licensee programs for probl_em identification or correction are unacceptable. In cases involving [ willfulness... breakdown in management controls, NRC intends to apply its full enforcement a hr y...up to the statutory 1imit of $100,0C0 @iolation Suggest $100,000 for this item alone, separate from other 0A identified problems. b. Need to know: How many people were involved? QC inspectors QC supervisors To what extent did the Bechtel QA organi~zation know of this practice? How high in the management chain was this practice known? Was this practice known outside of the Bechtel organization? ~ How long did this practice exist? Was this group audited by Consumers Power or Bechtel? Is re-inspection planned for those areas What were not fully inspected by Bechtel QC? m M y- ./. .+,

Items of nonconformance listed in the NOV do not present a strong case. 3 Examples are: s a. pg. 7 item p is not clear. b. pg. 7 item q is not clear. 2 c. pg. 8 item b - this statement is not clear that inadequate design review was done, d. pg. 9 item no. 3 Sounds very weak - poor case. e. pg. 12 item no. 5 Not sure what the problem is, but it sounds like a weak case, f. pg.13 item no. 7 - not sure that they were controlled drawings. Sounds like a weak case. 9 pg.14,15 item 8 - although not all items of nonconformarces were identified, it does not necessarily mean no corrective action was taken. 4 Ltr. page 2: A backlog of 16,000 inspections has been identified. Would like to have more information on this. Will they all be perforced? What kind of inspections are they?
