Midland - Units 1 and 2 Foundation Studies Enclosed is the first installment of material relating to your review of the Midland soil / foundation problems.. As I discussed with you on the telephone on March 20, I am sending you only portions of Chapters 1 and 2 of the Final Safety Analysis Report (FSAR) as well as Section 3.8.5 from Chapter 3.
A complete copy of the FSAR is maintained in the ACRS office and you are welcome to review it when you are in Washington. At some time in the future, we will arrange for you to visit the site and also to participate in one or more Subcomittee meetings. In the meantime, we will periodically send you additional material related to this review.

STOP WORK OF HEATING AND VENTILATING CONTRACTOR (MIDLAND SITE) on March 20,1980 at 4:00 p.m., consumers Power notified RIII (Chicago) that an order had been issued to the Zack corporation refraining them from doing any further safety related heating and ventilating work at the Midland site.
The order was issued because of problems associated with the failure of Zack to implement certain portions of the QA program. An NRC investigation during the week of March 17, 1980 confirmed the existence of the problems.
An Immediate Action Letter is being issued to confirm the stop work action and outline actions required prior to resumption of work.

Subject:
WELDING GAS DETONATION AT MIDLAND CONSTRUCTION SITE Consumers Power Company notified the resident inspector of a detonation of a reactive' gas used for welding operations at the Midland construction site.
Two pipefitters were attempting to find a gas Leak.
During the checks, they broke a hose coupling containing the gas at a pressure of 40 PSI.
The gas was apparently detonated by activation of a two way radio being used by a person near the hose.
ALL three men received second and third degree burns over fifteen percent of their bodies.
Initial prognosis is that the men's eyes and Lungs are not damaged. Some minor equipment damage occurred in the auxiliary building.
The event is being covered by Local news media.
The NRC does not intend to issue a press release.
The State of Michigan and OSHA are being notified.
Consumers Power notified the resident inspector at 2:40 p.m. on March 15, 1980.
This preliminary notification is issued.for information only and no further action by IE is anticipated.

This PN is an update of PNO-III-66 issued on December 20, 1979.
ru The following information was received by Consumers Power Company on tests performed on the Midland 1 reactor vessel anchor bolts.
1.
Test results indicate that most of the bolts have a hardness value above the ASME material, specification Limit (indication of brittleness).
2.
The cause for the two bolts previously reported broken at Midland 1 was due to stress corrosion s racking in conjunction with the potential sub standard mechanical properties of the bolts.
3.
The 96 bolts received by Midland may have been part of a larger order of bolts heat treated at the same time by the J. W. Rex Company. The disposition of the remaining 90 bolts from that order is not known at this time.
ALL information to date indicates that the bolt material in Midland Unit 2 is acceptable.
The Licensee is planning to meet with consultants and Bechtel next week to review the data further.
Region III wiLL follow tnis problem at the Midland site..
No press release is planned by the Licensee until the final evaluation of the r
material is made.
The NRC does not plan a press release at this time.

CPPR-81 and 82 is being issued to Consumers Power Company on December 6, 1979. This action is based on quality assurance deficiencies involving settlement of the Diesel Generator Building and soil construction activities at the Midland site, a material false statement in the FSAR, and an unresolved safety issue concerning adequacy of the licensee's actions to correct deficiencies in soil construction under and around safety-related structures and systems.
These matters do not involve an immediate threat to the health and safety of the public. A news release has been prepared and will be issued about the time the licensee receives the Order. The IE Director discussed this Order with the licensee on December 6, 1979. The State of Michigan will be notified by Region III (Chicago).
