Prepared By: WJClemens/DSiedlarczyk Initials WJP/QES Issue te07Q842, Appendix B Criteria 1NSTRUCTIONS, PROCEDURES & DRAWINGS Observation: 1) Pullman Power Products is in the process of welding a Unit #2 Reactor Building electrical penetration referenced on PPP ISO-2R72-26. The PPP QC inspector has already accepted the fit-up of FW #01 without having access to any drawing except the Pullman isometric. This iso does not contain fit-up criteria for subject field weld. This criteria is found on Westinghouse Dwg. #E-40047, Rev. E.

2) PPP WPS IT12A does not address weld joints made by open butt detail with backing as is the configuration on the electrical penetrations.

N Review for Significance:

EI Justifica! Ion: PPP QA Manual,Section V, Paragraph 5.3.1, "All field fabrication and installation shall conform to Customer furnished drawings, specifications and instructions."

Recommendation:

(1) Pullman Power Products to nenerate nonconformance report covering above condition.

(M Pullman Power Products to nrovide accessability of reference drawings to field norsonnel-(3) PPP to reindoctrinate personnel involved with above condition.

(4) PPP to utilize a WPS which clearly reflects the weld configuration in the field. ,

CORRECTIVE ACTION REPLY:

The Westinghouse Dwg. #E-40047 has been removed from the Field Engineer's trailer and is now in the foreman's work station. PPP has contacted Coastock. and

, they will provide additional drawings as soon as possible. Pullman does not feel that any of the work that has already been complete to date would constitute an NR condition.

4 All welding has been controlled per PPP's WPS and GWS. In addition, there will be a full volumetric inspection (UT) of the final weld to verify quality.

1) CQE has initiated NR #CQC 2538 to address the nonconforming condition.
2) CQE has verified the placement of Wes tinghouse Dwg. #E40047, Rev. E, in the work area, thus providing accessability to field personnel.
  • 3) PPP to provido to CQE documented evidence of personnel training in the area of utilizing proper drawings.
  • 4) Per discussions with Project Organization (welding) Design Engineers, it has been determined that the Pullman Power Products WPS #1T 12 A does not adequately address the joint configuration as detailed by Westinghouse drawing #E40047. The electrical penetration joint detail is one of an open butt, full penetration weld with backing. In that the PPP WPS does address the open butt portion of this configuration, Project Organization has no concern to this point. However, in no viable way is backing on a joint identified under WPS IT 12 A or the supporting CWS sub paragraphs.

, It is this fact which concerns P/O Engineering and CQE.

ASME Section IX, Table QW-256, Gas Tungsten Arc Welding lists under the heading "256.2 Nonessential Variables", the following:

QW-402.5; (The addition of backing and its nominal composition.)

ASME Section IX, QW-201.2, second paragraph states:

" Changes may be made in the nonessential variables to suit production requirements without requalification of the procedure, provided such changes are documented with respect to both essential and nonessential variables of the specific welding process or processes, either in an amendment to the original WPS, or a new WPS."

With the requirements of the Code in perspective, PPP shall rectify WPS IT 12 A so as to reflect the actual weld configuration in the field.

However, after review of both the subject AR and your memo, it is clear to me and the AR initiators that the action ,to be taken, as presented by yourself on said memo, had already been considered and so documented. The need for a non-c.onformance regart addressing the condition of the electric penetration of AR #493 was not neglectedlf CQE as was inferred by your memo of 7/8/82.

You may wish to re-review the recommended actions of AR #493 of which #1 read:

" Pullman Power Products to generate nonconformance report covering above condition."

Pullman Power Products has responded negatively to recommendation #1, at which time CgE initiated NR CQC 2538.

According to program requirements and as discussed in recent AR Training, an AR cannot be written to address hardware problems. Since this is a hardware problem, it should have been addressed on a nonconformance report. This is necessary to avoid a recurrence of the problems identified in the NR task force assess-ment.

Please take the following actions:

1. Issue an NR to address the concerns of this AR or have Pullman issue it.
2. Close out this AR by referencing the nonconformance report.
