CIO ACTIVITIES
}
A.
Summary
- 1) Review of architectural documents processed through the register reconciliation phase, Phase III, of the FCR/FCN Resolutica Program.
- 2) Review of Civil Documents processed through the resolution phase, Phase II, of the FCR/FCN Resolution Program.
- 3) Evaluation of Construction Non-Manual Training Records.
- 4) Attendance at meetings.
B.
Details (Paragraphs below correspond to items above)
- 1) To verify the adequacy of steps taken in the register reconciliation phase, Phase III, of the FCR/FCN Resolution Program, Architectural documents assigned to four stations were sampled. Stick files were checked to ensure that the documents selected were present. Also, the documents were checked for consistency with the document registers.
No discrepancies were found with documents that had been processed through the FCR/FCN Resolution Program. One controlled drawing, which was not affected by and FCR or FCN and, therefore, not processed through the Resolution Program, was missing from a stick file. This discrepancy was documented by NIR 009.
Similar checks will be conducted using documents from other disciplines when those documents become available.
i
o JOB NO. 14509 MIDLAND NUCLEAR POWER PLANT REPORT NO. 27 PAGE 2 I
- 2) To verify the adequacy of steps taken in the resolution phase, Phase II, of the FCR/FCN Resolution Program, civil drawings were sampled. Resolution sheets for the FCRs and FCNs associated with those drawings were reviewed. Results of the sampling were j
satisfactory, although one FCR was rejected. Resolution sheets
}
for that FCR did not indicate clearly whether one or two drawings were affected by the change. This discrepancy was documented by NIR 010.
Similar checks will be conducted for other disciplines when those documents become available.
i
- 3) To verify the adequacy of steps taken to correct the training records of construction non-manual personnel, ten records that had been processed through the corrective action program were evaluated.
No discrepancies were found.
Additional records will be reviewed as they become available.
- 4) Meetings attended are summarized in Attachment A.
l l
NEW CIO ITEMS t
- 1) Tracked Action Item - #39 To check the adequacy of the register reconciliation phase, Phase III, of the FCR/FCN Resolution Program, a sample of controlled architectural documents Peld at four stations were reviewed. Although no discrepan-cies were found with documents that had been processed through the Resolution Program, a drawing that had not been processed, A-213 at QC Station No. 55, was found missing from a stick file. NIR 009 was issued to document the deficiency. MPQAD added drawing A-213 to the file and a re-sampling was conducted. That check was satisfactory and the item and NIR were closed.
- 2) Tracked Action Item - #40 To check the adequacy of the resolution phase, Phase II, of the FCR/FCN Resolution Program, a sample of civil drawings was reviewed. Resolution sheets for one' FCR, FCR C-1526, were found to be ambiguous. The sheets were unclear as to whether the FCR affected both of the drawings listed, C-358 and C-479. Field project engineering has issued another resolu-tion sheet, which is being reviewed.

The FCR/FCN C-1526 did not indicate which drawings, whether it be C-358, C-479 or both, were affected by this change. Subsequent resolution sheets did not clarify this situation. Due to these facts CIO concludes that a potential exists for FCR C-1526 to be misapplied to the drawings.
