Assessment Team - Observations With the Stop Work Order still in effect and since MPQAD has not completed the Phase I review and release of the FCR/FCN issue resolution, the Assessment Team has no observations on the site activities for this time period.

The Team members continued to review Work Activity Packages and deal with the issuing and resolution of various NIRs.

Work Activity Packages Work Activity Package 59 comments have been satisfactorily resolved with WAP 71 remains open, awaiting _re_sponse the final approval of the FCR.

WAP 69 was reviewed and comments were submitted to to one action item.

DISCUSSION Status Item Item 63 Rejacking at E8 Grillaga.

J.

Fisher reported that the E8 Grillage required rejacking on 11/23

-and again on 11/27 because movement reached a,lert levels.

Currently, the jacks on the grillage are being held at a load approximately 15 %

higher than the previous lock offload _.in order. to limit future movement.

- New Item Item 63 Non-Structural Weld Inspection.

Majeski clarified the concern raised by the Assessment Team in Weekly P.

Report 60, pg. 2.

He indicated that the Assessment Team believes that the base metal for non-stru'etural welds on permanent steel should be inspected by MPQAD. Steps should be taken to assure that this will occur. (INFORMATION ITEM, Item WR 60, pg. 2 Remains Open )

Fisher acknowledged the Team's concerns regarding the mixing and consistency of the bentonite slurry.

He indicated that if similiar work was performed in the future, these concerns would be considered during preparation of the procedure.

Majeski said that he had reviewed the drilling procesure Torinstallation.of the soldier piles and concurred Since there with FSO's previous conclusion that a change is not required.

are no plans to drill additonal slurry supported holes, other than those covered under the soldier pile installation procedure, the Assessment Team considers the discussion of the item closed.

Fisher reported that the load on the grillage has been maintained _at.

about 15% above the previous lock off lord. It is expected that the settlement -

rate criteria will be met by tomorrow at which time the load will be' reduced to 135% of design loac (previous lock off load was 125% ).

Item 63 Project Engineering Evaluation of Welding.

Fisher supplied a copy of the report-the project welding engineer prepared J.

The report contains numerous observations on changes to earlier this year.-

drawings or procedurea. The Assessment Team will close its' action item based on the submittal of this report. Further evaluation of welding by the Assessment Team will be done periodically as part of the general on-going assessment. ( CLOSES ITEM WR 30, pg. 3 )
