ATTACHMENT TO LER-78-089/03L-0 Page 1 of 1 While shutdown for refueling and maintenance, Plant Management noted that the surveillance testing required by Technical Specification Appendix A, Paragraph 4.12.E.1 had not been performed for those combustion ioni:ation products (smoke) detectors that were installed in the diesel fire pump room as part of a modification on March 10, 1978.

Had the detectors been inoperable, a fire in the diesel fire pump room might not have been detected until the wet pipe sprinkler system which pro-vides fire suppression for that area was actuated.

Following the issue of Technical Specification Amendment No. 34 (Fire Protection) in January of 1978, a listing of those Surveillance Tests which would be required was drawn up. Since the smoke detectors were not installed in the diesel fire pump room at that time no Surveillance Test for those detectors was proposed which resulted in the oversight. A new Surveillance Test (F-ST-76J22 titled Smoke and Heat Detector Functional Tests - Diesel Fire Pump Room) was written and performed as soon as practicable following discovery and the components were found to be operable.
