w On June 15, 1982 we received a Chem-Nuc r cask, CNS N serial number P/I-20489, for shipment of Type B or large quantitles o

'". J radioactive material. The certificate of compliance number is No. 6601, Revision 9.

On Thursday, June 17, the bolts from the top shield cover were removed to the hot shop for cleaning per procedure. The first bolt cleaned showed a crack the length of the bolt. Consequently, a mag particle test was performed on all 32 bolts. The results showed 14 additional stress marks, either on the bolt head or on the threaded portion.

Chem-Nuclear was contacted and delivered 18 new bolts on Monday, June 21. These bolts all passed the mag particle test at our site that day.

The cask left site on Tuesday, June 22, with the new bolts in place and the 15 questionable bolts accompanying the shipment for review by Chem-Nuclear.
