JUL 9 1987 MEMORANDUM FOR:
.KarlKniel,' Chief.4 Reactor and Plant Safety Issues Branch Division of Reactor and Plant Systems, RES THRU:
n Paul Norian, Section Leader l' V Reactor and Plant Safety Issues Branch, DRPS/RES FROM:
Aleck W. Serkiz, B-56 Task Manager Reactor and Plant Safety Issues Branch, DRPS/RES
SUBJECT:
REPORT ON EDG RELIABILITY PROGRAM MEETING A meeting was held on June 23, 1987 to review the EDG reliability program being developed for resolution of generic issue B-56.
The participants are shown in the enclosed attendees list; the meeting agenda is also enclosed.
The draft documents discussed were:
Diesel Generator Reliability Program Review Guidelines (Rev. 2, dated 6/17/87) and Appendices A, B, C and E.
Although there was a consensus regarding the critical review elements of a l
suitable diesel generator reliability program and the approach being taken in developing this EDG reliability program, the definition of diesel reliability, unavailability target values and calculational methods were not resolved at this meeting.
The continued use of RG 1.108 was not supported by persons present.
The extent and scope of discussions can be derived from the enclosed meeting notes taken by Diane Noel, RPSIB.
The result of this meeting was to identify where major rewrites are necessary (namely Appendices A and C) and the need to assemble a complete document which includes all appendices to facilitate a complete review.
That effort is
{
underway and I expect a complete report from SAI by July 24, 1987.
I plan to use the July 1987 draft to formally request a review by NRR staff.
The report which is emerging will also be of use in revising SRP sections and Regulatory Guides pertinent to both B-56 and USI A-44.

If you have further questions regarding this meeting I will discuss them with you at your convenience.
By copy of this memorandum, I am forwarding the working drafts used at this meeting to the PDR.

EDG Reliability Procram (Ref. Issue B-56T i
Review Meeting Agenda 6-23-87 General Objectives:
1.
Review EDG Reliability Program Guidelines (6-17-87 Draft) and Appendices A, B, C and E.
2.
Obtain NRR needs, views and recommended changes.
Specific Objectivest 1.
Obtain consensus on the main elements of an EDG reliability program (RP). See Table 1, Pgs 1-3 &
1-4 of Guidelines text.
2.
Review Sections 2 and 3 and obtain consensus on approach and scope of each of the main elements.
Key questions are:
A. 'Is this what the NRR reviewer needs ?
B.
Is this approach' consistent with the better EDG reliability programs at plants ?
C.
Would this approach be useful to the plant community at large 7 D.
Is this form and content of.use to l
EDG maintenance staff 7 3.
EDG reliability and/or availability targets or goals:
j j
Key questions ares l
A.
Is Section 3.1 written correctly to assure compliance with the propose &RG I
for USI A-44 7 If not how should it.be changed?
B.
Is Appendix A understandable and will it be consistent with Regulatory in ef f ect 7 requirements (or SRP guidelines)-
Examples: SRP sections B.3.1 8.3.2, IEEE-387, RGs 1.9,I.81,1.93, RG 1.108, etc.
I C.
Is Appendix A consistent with-INPO's di esel generator unavailability data reporting instructions?
Lunch Break l
4.
Appendices B,C and E These Appendices will need to be reviewed for consistancy with the results of the i
morning discussions and acceptablity to j
NRR staff.
Adjournment: 4:00 PM Target Thanking you in advance for your participation and positive contributions.

Meetina Notes from EDG Reliability Proaram on 23 June 1987 by den l
SPECIFIC OBJECTIVE 1 (see Meeting Agenda),
- need to get root cause on e)-
t
- preventative maintenance focus is not. clear on d)
- common cause is not clear - bring it out as a separate el ement
- need terms of meeting reliability target
- EDG maintenance is being turned over to. mathematicians
- different maintenance practices can contribute to system
- reliability program is needed to give f eedback from each i
plant
- what happens when you drop below acceptable goal?
- common cause should be used as' indicators
- this is really an operational reliability program i
SPECIFIC OBJECTIVE 2 (see Meeting Agenda)
- this not only to be applied to those non compliant with.
the goals--should allow prediction of EDG failure,.a sort
{
of auditors checklist utility is doing well'(recording perf ormance
- if a objectives--this is what we are asking(develop a trend))
- elements should be reviewed by auditor
- preventative' maintenance is not mentioned
- spare parts is not mentioned
)
SPECIFIC OBJECTIVE 3 (see Meeting Agenda)
Question A
- delete reference for run time l
- what do we mean by target? - need.a method to know if i
meeting target
- reliability target has not been stated l
- need to translate target into performance
- target 'is long term goal to let us know when wd're getting into trouble
- more a measure of how reliability program is' working
- should be consistent with regulation guide for A
- utilities have been preparing for EDG tests l
- use as " considerations" for interpretating the regulation guide

l l
I
- target should be consistent with regulation
- redefining f ailures and diesel starts, first and last bullet are inconsistent with Regulation Guide-1.108 (concentrates too much on the numbers, should be withdrawn)
- A-44 and 1.108 are not consistent Bullets in Section 3.2
- 1ast bullet:
conditions not typical - change to "from the control room" l
l SPECIFIC OBJECTIVE 3 (see Meeting Agenda)
Questions B and C
- mission time inconsistent
- page 10:
need simple, straightforward approach
- intents to check for validity
- coping time is not necessary
- one hour EDG test too short - need to be thermally stabilized
- mission time should be considered for a valid test need relationship between. common cause and independent f ailure l
- needs elements of 84-15 j
- regulations want controls not monitoring I
- provide incentive f or a correctly run program
- NRC only looks at performance - regulations should.be written up so plants don't cover up failures
- failures need to be put in reliability. requirements l
- looking for " cook book" formula which doesn't have to be i
based on catastrophic failure
- EDG reliability guide should include frequency and o
l increases in severity I
- " tripping the indicator..." is to easily abused
)
l
- tests should not be extended to do damage to the EDG l
(operators have EDG problem, but continue to run EDG a
in order to pass test)
- give specific guidance to which-failures they should count i
EDG Reliability Program Element - AWS Draf t
- add spare parts to item-D.
- need to define. triggers for regulatory detention
- add INPO as base for interim in Appendix A l

SPECIFIC OBJECTIVE 4 (see Meeting Agenda)
Appendix B
- objective of reliability program - to make EDG work
- audit to see if licensee following intent of report
- need indicator to tell if program is working
- flow diagram is needed
- checklist is needed
- up to licensee to tell them Appendix E
- give them instructions, examples, and then let them develop their own reliability program
- only address failure investigation as part of a root cause analysis
- whc; happens when plants have a common generic problem but no one has done anything about it?
- relates only to design portion of chart
- do a root cause analysis if:
a) has i t occurred bef ore?
b) has it occurred under the same mechanism
- there is a difference between root cause analysis and f ailure investigation
- for every failure go through a failure cause analysis, if it is a systematic probl em, then do a root cause analysis
- role of the vendor part of the failure and root cause anal ysi s
- Problem Vendor Generic v
Failure Cause Analysis 4
J Is this Systematic yes no ---
h Root Cause
- put failure analysis under root cause analysis DOCUMENT AS A WHOLE
- need to change operational documents
- test against known perturbations
- aging not recorded in this report
