Hydrocen Builduo in Waste Gas Di scharoe Header h/f y Q SQ 'was assigned to perform a calibration in 1986 on ractation channel 9095, which monitors the waste gas discharge header.

After i sol ati ng the moni tor from the header and while injecting the test radioactive gas, a small hydrogen detonation occurred insice the sample flow path within the instrument cabinet on the -25 ft. el of the auxiliary building.

Although no equipment damage occurred, the occurrence was significant because (i) it demonstrated a hydrogen control problem existed in either unit 2 vent gas header or in the interface with unit I on the discharge path to the stack; and (i i ) the potential for more adverse consecuences existed.

The occurrence was di scussed with ILC supervision and plant management.

Actions were taken to add a caution to the I&C calibration procedure and to add a recutrement in an operatic s procedure to purge the header prior to the ILC calibrations.

concern on whether the radiat2 cn monitor provides representative indication of SJAE radioactivity is based on the repair history for the monitor.

The monitor malf unctions af ter each plant trip aid chemistry because the sample lines flood out.

I analysis of chemistry samples.

said the system has received management at t en ti on and design changes have been made to reduce the moisture problem.

However, believes further design changes are needed to rolocate the sample line tap off point closer to the condenser on the air ejector discharge, which is where the local chemi stry sample point is located.

My review of Unit 2 tech. specs. 3.3.3.9 shows that either the SJAE or the steam generator blowdown radiation monitors is required to be operable as an LCO to i sol ate the steam generator blowdown in the event of tube leak.
