e ENCLOSURE ALLEGATIONS AND ACTIONS Issue 70-1:

j During a recent condenser backwashing evolution on April 12-13, 1991 the "C" circulating water pump was tripped by operators. Concurrently, the "D" circulating water pump inadvertently tripped. A plant incident report was not written describing this event. In addition. During the past refueling outage (October 1990) a similar event occurred with no 3

subsequent plant incident report.

Action:

This issue has been referred to the licensee for their review and action. You will be informed of the results.

Issue 70-2:

The NRC is not properly addressing OSHA concerns about inadequate work space around several electrical panels.

1 Action:

This issue has been referred to the OSHA Region 1 Liaison for review and action.

Issue 70-3a:

Charging pump "A" is a Category I EEQ component. However, the junction box, containing the electrical connections to the motor, did not have a cover gasket. This condition is not in compliance with acceptable EEQ configurations.

Action:

1 This was not substantiated by our review. However, since the junction box utilizes qualified EEQ splices within the box, the issue has no safety implications, and is therefore closed.

Issue 70-3b:

It was noted, during a preventive maintenance on the spare boric acid pump motor located in the warehouse, that the motor heaters were not energized to keep the windings at least 5 degrees F above ambient as is required on the PM card.

Action:

This issue has been referred to the licensee for their review and action. You will be informed of the results.
