EVENT DESCRIPTION

On October 21, 1982, at 0110 hours0.00127 days <br />0.0306 hours <br />1.818783e-4 weeks <br />4.1855e-5 months <br />, Unit Two was at 807 MWe, and monthly surveillance QOS 1600-1, Suppression Chamber to Drywell Vacuum Breaker Test, was being perforned.

During this test, vacuum breaker 2-1601-33A stuck in the open position. As required in Technical Specification 3 7. A.4.b, surveillance procedure QOS 1600-2, Suppression Chamber to Drywell Pressure Separation Test, was performed. The differential pressure criteria in this specification could not be satisfied, there-fore, an orderly shutdown of the unit was initiated, at 0950 hours0.011 days <br />0.264 hours <br />0.00157 weeks <br />3.61475e-4 months <br />, as per Technical Specification 3 7.A.6.a.3 The incident was declared an Unusual Event as defined in the Generating Station Emergency Plan, and the NRC was notified. The unit was manually scrammed at 1702 hours0.0197 days <br />0.473 hours <br />0.00281 weeks <br />6.47611e-4 months <br /> and was at zero pressure by 0700 hours0.0081 days <br />0.194 hours <br />0.00116 weeks <br />2.6635e-4 months <br /> October 22, 1982.

VI.

PROBABLE CONSEQUENCES OF THE OCCURRENCE:

With the vacuum breaker stuck in the open position, the designed differential pressure between the Suppression Chamber and the Drywell could no longer be maintained.

As a result, there was an increased volune of water in the downcomers.

In the event of a postulated loss of coolant accident or the lif ting of a safety valve, larger slugs of water would be forced out of the downcomers. The greater dynamic forces imposed on the Suppression -Chamber and its associated piping by these larger slugs of water may have resulted in Suppression Chamber damage.

Immediately af ter the vacuum breaker was found to be inoperable, the appropriate surveillance was initiated as required in Technical Specification 3 7.A.4.b.

The differential pressure criteria stated in that specification could not be met.

Therefore, as per Technical Specification 3.7. A.6.a.3, an orderly shutdown of the unit was initiated.

The prompt unit shutdown minimized the consequences of this occurrence.

er Vll. CAUSE:

The stainless steel packing and stuffing box bushing were bound on the valve shaft.

This caused the valve to be stuck in the open position.

Vill. CORRECTIVE ACTION:

The valve was f reed from its shaft and properly lubricated.

It was then exercised several times without -further incident. As a p recau t ion, the other il Drywell to Suppression Chamber Vacuum Breakers were also lubricated and exercised.
