SUBJECT:

FAILURE OF TARGET ROCK SOLEN 0ID VALUE DURING ENVIORNMENTAL QUALIFICATION The writer called Mr. Richard Rock of United Engineers on Mr.rch 10, 1982 regarding the subject failure.

United Engineers is the AE for WPPSS.

Target Rock is performing the qualification on their solenoid valve per UE contract.

The NRC has been following the Target Rock test program, initiated at a meeting held at Richland Washingtion between WPPSS, UE, NRC (NRR, RIV, Sandia) on April 21-23, 1981.

.2 The results of our inspection activities at Richland are contained in the inspection report for Dockets Nos. 50-460/513 dated July 1, 1981, a copy of which is attached.

American Environments, East-West Laboratories conducted the LOCA test.

NRC representative (A. Finkel, RI, R. Agee, RIV) and Sandia personnel witnessed various portions of the LOCA/ pre-LOCA activities.

Although a formal report has not been issued to date, some of the observations at East-West Laboratory were:

1.

Test instrumentation failed and was inadequate or non-existant for monitoring ~important parameters such as chemical spray flow rate or temperature.

The only test specimen was damaged during maintenance periods (LOCA test interrupted to repair failed components, such as heaters located in the test chamber).

Baseline data was not recorded.

4.

No back up instrumentation or test specimens were available.

The failure'of the solenoid valve occured somewhere between 31-38 days post-LOCA.

They were cycling the value every 7 days, the last successful cycling occurred 31 days post-LOCA.

The failure was observed during the attempted cycling on the 38th day.

During earlier discussions with R. Agee/RIV and myself, one of the reasons for failure was given as contact corrosion of the rectifier tabs between the normally installed rectifier and on alternate rectifier connected in series subsequent to the damage that occured during a maintenance procedure.

Since it was inferred that the problem resulted from different material make-up between the two rectifiers, the quality control aspect of this program was discussed.

UE's contract to Target Rock imposed the QA requirements of the contract to

the vendor (Target Rock) and sub-vendor (East-West Laboratories).

In earlier j l

discussions between Mr. Agee and WPPSS personnel (Rhodes, et. al.), it was disclosed that WPPSS had not audited East-West Laooratories.

WPPSS message, I

to paraphrase was "We selected our AE and expected them to pick up the, ball, we can't follow each program."

I have asked UE (Rock) to consider the following:

1.

Generic implications resulting from the solenoid valve failure.

(Mr. Rock advised that none of the valves are installed in WPPSS at this time.) He stated that the generic consideration was Target Rock's responsibility since the UE contract is limited to WPPSS No. 1.

2.

The impact of the failure on the WPPSS contract.

A meeting is scheduled between WPPSS, UE and Target Rock the week of March 22, 1982.

I will contact WPPSS to arrange a meeting in Bethesda the week of March 29, 1982 to:

1.

Formally brief us of the particu'ar failure and plans for future testing.

2.

The generic impact of Target Rock valves of the design that experienced the failure.

NRR/EQB and RIV' personnel will be invited to participate in this meeting.

Based upon the foregoing, we should perform a more detailed review of the WPPSS/UE enviornmental qualification program.
