HIGHLIGHTS OF WEEKLY INFORMATION REPORT Week Ending June 24, 1983 04ic: cf Administration

Daring the period June 13 through 24, 1983, the Occupational Safety and Health Staff conducted a series of ten one-half hour training sessions for NRC Head-quarters employees on the specialized subject, " Video Display Terminals...The Human Factor." One hundred sixty-two NRC Headquarters personnel attended these training sessions. After June 27, the training film will be available for loan to NRC superviscrs by request to the Safety and Health Staff at x27934.

Status of High-Level Waste Disposal Contracts In response to Section 302 of the Nuclear Waste Policy Act, 41 high-level waste disposal contracts have been signed and received by DOE as nf June 22, 1983.

The remaining 31 contracts are expected by DOE to be signed on or before the statutory deadline of June 30, 1983.

Significant Enforcement Action A Notice of Violation, EN-83-42, and Proposed Imposition of Civil Penaltics in the amount of One Hundred Fifty Thcusand Doliars ($150,000) was issued on June 21, 1983 to Ce^m:m..eolth Edison Company. This action was based on three Severity Level III violations involving shutting down the reactor by inserting control rods in tte reverse order than that established by procedure.

Naticnal State Programs Meeting Cver 100 persons attended the second National Meeting en Low-Level Radioactive Waste (LLW), June 20-21, 1983, in Baltimore, Maryland, representing the compacts, States and Federal agencies.

The meeting was sponsored by NGA, Southern States Energy Board and D0E. Workshop discussions centered around three policy issues:

(1) Post-1986 waste management options; (2) State siting procedures; and (3)

Federal waste provisions and uniform definitions in the compacts.

ITEMS OF INTEREST - JUNE 24, 1983 CHANGE IN THE PRIORITY PLACEMENT LIST Under the FY-83 management plan, individuals who occupy overage positions in staffing plans are identified on the Priority Placement List and are considered for placement in vacant NRC positions for which they qualify.

Since the last report, two additional names were deleted.

Since October 13, 1982, a total of 78 names have been deleted.

Of these, 51 were placed through internal reassignment, 8 have left the Agency, and 19 were selected for internal vacant positions.

A total of 65 employees remain on the Priority Placement List.

During the period June 13 through 24, 1983 the Occupational Safety and Health Staff conducted a series of ten one-half-hour training sessions for NRC headquarters employees on the specialized subject, " Video Display Terminals...The Human Factor."

The short presentation included the showing of a National Safety Council film by that title, a statement identifying available studies which document findings and conclusions regarding radiation safety and the VDTs, and an update of the progress made within NRC to address some of the ergonomic design aspects and adjustments of VDT work stations. One hundred and sixty-two NRC headquarters personnel attended these training sessions.

After June 27, The training film will be available for loan to NRC supervisors by request to the Safety and Health Staff at extension 27934.

OFFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending June 24, 1983 l

" Regulatory Reform Case Studies" Maeting On June 27,1983, from 1:00 to 3:30 p.m. in the Phillips Building, Room P-118, Mr. John Crowley, Manager, Advanced Engineering, United Engineers &

Constructors, will present the highlights of his firm's work on tracing examples of NRC regulatory requirements for nuclear power plants through their industrial interpretation, with a focus on cost and productivity impacts.

The United Engineers' work was done in support of Argonne National Laboratory's contract with the Department of Energy aimed at developing recommendations for improvements in the nuclear regulatory process.

The technical contact is George Sege (X24609).

Lacrosse Boiling Water Reactor On June 20, 1983, LACBWR was given a one-time exemption from the requirement for annual emergency preparedness exercises which allows the next exercise to be delayed from August 1983 until December 1983.

McGuire Unit No. 2 Unit No. 2 was shut down on June 17, 1953 to modify the preheater section of each Model D-3 steam generator. The outage is scheduled for approximately 10 weeks.

Prior to the shutdown, the unit had operated up to 30% of full power since issuance of the full power license on May 27, 1983.

Extended Outage of the Indian Point Nuclear Generating Plant, Unit No. 3

On Saturday, June 18, 1983 at 8:30 p.m. there was an electrical failure of the main turbine generator at IP-3.

At that tinte the licensee commenced plant shutdown.

The licensee and Westinghouse are presently investigating the cause of melted windings.

The outage is expected to be at least 18 weeks in duration.

Revocation of LWA-1 and Withdrawal of Application for Ccnstruction Permits for the Black Fox Station, Units 1 and;2 By letter of June 22, 1983, to the Public Service Company of Oklahoma, the Director, Nuclear Reactor Regulation, revoked the LWA-1 for Black Fox Station, Units 1 and 2 that was granted July 26, 1978, and amended on September 6, 1978, November 30, 1978, and July 24, 1979. A copy of a notice of withdrawal of the construction permit application and termination of the proceeding was enclosed with the letter.

. The March 7,1983, Licensing Board order granting the Applicant's request for withdrawal of application and termination of the proceeding was conditional that subject to NRC staff's monitoring and approval, (1) the applicants implement their soil stabilization and erosion control plan by no later than September 24,1983, and (2) dismantle those site improvements not to be utilized at the Inola Station (coal), to be constructed on the site, in such a manner as not to cause any onsite or offsite detrimental environmental impacts.

l NRC TMI PROGRAM 0FFICE WEEKLY STATUS REPORT June 19-25, 1983 AUXILIARY AND FUEL HANDLING BUILDING ACTIVITIES:

The major ongoing activity in the Auxiliary and Fuel Handling Buildings (AFHB) continues to be the decontamination of the 282' elevation.

The scabbing activity is now centering on individual cubicles since most large open areas and hallways are completed.

No remote decontamination was performed since the robot (Fred, described in Weely Status Report dated May 23, 198') experienced maneuverability problems during its last use and is now scheduled to be decontaminated and repaired before reuse.

Cleaning of the Auxiliary and Fuel H.andling Building floor drains continued this week.

Four chemical decontamination mixtures containing phosphoric acid are to be tested on steel pipes and supports on the 305' elevation in the FHB annulus.

These w'ill be evaluated for decontamination effectiveness.

Routine trash compaction and tool separation / decontamination continued in l

support of other tasks and activities presently in progress.

The FHB crane was returned to service on a periodic basis to permit performance of tasks such as the weighing of SDS liners.

The remaining maintenance items on the crane and rails will be completed by July 5,1983.

E'xpansion of the decontamination facility continued with the installation of the jib crane in the clean area of the facility.

Other work included electrical wiring and continued work on the shell structure.

The duct work is in place for the ventilation system.

Supporting procedures governing the operation and use of the newly purchased decentamination equipment are still in the licensee review process. '

REACTOR BUILDING ACTIVITIES:

1.

Entries.

Four Reactor Building entries were completed during the week of i

June 19, 1983.

The high pressure hose which supplies decontamination flush water to the Reactor Building was repaired and decontamination.

activities resumed inside the building.

Cecontamination efforts were-focused on the interior surfaces of the LOCA ducts and on the open stairway between the 305 and 347 ft. elevations.

In both cases a high pressure. rotating flush nozzle was used to decontaminate the surfaces

-remotely.

Four reactor building entries have been scheduled for the week of June 26, 1983.

Reactor Vessel Underhead Radiation Measurements. The radiation profile i

taken inside the reactor vessel (Quick Scan One) in December 1982, indicated that the dose rate profile between the reactor vessel head and I

the plenum. ranged from 40 to 600 R/hr.

It had been assumed that indicated dose rates were due solely to more highly penetrating gamma radiation instead of a combination of gamma and softer, less penetrating beta.

However, an NRC prompted beta response test of the detector used during Quick Scan..One revealed that the beta efficiency of the instrument to a 40 mci Sr-90/Y-90 source was as high as 36%.

2 centribution to the Quick Scan One radiation measurements cannot be evaluated.

This is due to compounding factors including; unknown beta source terms on the underhead components, water shielding effects and the uncertain distance' of the detector relative to the beta sources on the control rod guide and support tube surfaces.

Quick Scan Two, the next phase of underhead radiation measurements, will incorporate a beta shield over the detector to obtain gamma dose rates inside the reactor vessel.

The center control rod drive opening will be used during the Quick Scan Two experiment and radiation measurements will be taken from the top of the reactor vessel to the top of the core debris.

These measurements are. scheduled to be collected in August 1983.

POLAR CRANE STATUS:

The NRC continues to review Reactor Building 5-ton hoist procedures for miscellaneous tasks in containment.

Addendum I to the Polar Crane Load Test Safety Evaluation Report (SER), dated June 17, 1983, was received by the staff for review and approval. The Load Test Procedure for the Polar Crane was also received by the staff on June 20, 1983, for approval.

It is anticipated that the Load Test Procedure and Operating Procedure will not be formally approved by the staff until the NRC approves the licensee's SER.

None of the above approvals will occur until the NRC's Office of Investigation's review of the polar crane al, legations is completed.

The licensee is continuing to analyze the significance of undersized welds that have been discovered on the Head and Internals Handling Fixture.

The NRC staff will review the licensee's findings and determinations.

DEFUELING'FREPARATION ACTIVITIES:

Refurbishment and preparation of the "A" spent fuel pool for the eventual staging and temporary storage of core fuel and decris continued this week.

The

~

upper tank fill and drain flushing process is presently on hold with 3 feet of water retained in the tanks for shielding purposes.

General area radiation levels remain at 35 mR/hr.

The fill and drain process will continue as water inventories permit.

Lif t lug welds on the shield blocks coverirg the tanks are presently being' inspected and will be load t'ested soon.

The SER covering the heavy load drop and tank removal (presently being prepared by GPU) is scheduled for NRC review beginning in early August.

In support of the eventual underhead characterization and headlift, the NRC is presently reviewing numerous operating procedures related to RCS drain down and underhead_ characterization tasks.

NRC is awaiting licensee response to 14 items needing further clarification with respect to the SER submitted for underhead characterization.

The licensee intends to complete all software related to underhead characterization prior to starting RCS drain down.

It now appears that drain down will be initiated sometime in mid-July.

SDS Liner Shicments.

Tne inieakage was attributed to catalyst pellet bic:Lage of both the Hansen check valve and mechanical plug on the catalyst insertion vent line.

The mechanical plug was removed and the liner vent tool was reinstalled in order to clear the pellets.

The liner will be redewatered and vacuum dryed. This drying process will take approximately 10 days.

Liner D10011, which contains approximately l

88,000 curies, is not scheduled to be shipped until July 5, 1983.

As with previous SDS shipments. preparations, liner D10011 will be monitored to assure against combustible gas mixtures during handling and shipment.

2.

EPICOR II Prefilter (PF) Shioments.

One EPICOR II prefilter (PF-12) was shipped from TMI to the Idaho National Engineering Laboratory (INEL) on June 23, 1983.

This shipment represents 45, in a group of 50 liners, that have been sent to INEL over the past year.

Three prefilters (PF-4, PF-21 and PF-22) are scheduled for shipment next week.

The prototype gas sampler, which is used to sample and inert each liner prior to shipment, was modified to compensate for the raised vent plugs on the final four EPICOR 11 prefilters.

This repair was completed and the sample chamber was satisfactorily leaked tested prior to sampling PF-21.

The licensee anticipa(es the last EPICOR prefilter will be shipped offsite by July 13, 1983.

SCHEDULE OF FUTURE ACTIVITIES:

1.

Primary system depressurization and craincown is tentatively scheduled for mid-July.

2.

Plans to sample the reactor coolant drain tank sludge vill begin, at the earliest, in six weeks.

Characterization and removal of this material will follow.

3.

P'lans to sample and characterize the Reactor Building sump sludge are also under consideration for this summer.

4.

OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS ITEMS OF INTEREST Week Ending June 24, 1983 Canonsburg, Pennsylvania In a meeting on June 17, 1983, DOE representatives briefed NMSS staff members on the Remedial Action Plan (RAP) proposed for the Canonsburg site. After the formal briefing, NMSS technical review personnel met with DOE and DOE contractor counterparts to discuss the initial NMSS review of the RAP and to inform DOE of the additional infonnation, as identified in the initial review, required to continue the technical review of the RAP.

DOE and NMSS staff members also discussed, and agreed to, changes in the Final Environmental Statement for Canonsburg which will satisfy NRC comments to DOE concerning recognition of the Th-230 contamination at Canonsburg and criteria for site cleanup.

Status of High Level Waste Disposal Contracts In response to Section 302 of the Nuclear Waste Policy Act, 41 high level waste disposal contracts have been signed and received by DOE as of June 22, 1983. The remaining 31 contracts are expected by DOE to be signed on or before the statutory deadline of June 30, 1983.

US/IAEA Safeguards Agreement On June 15 and 16, representatives from the Safeguards Division participated in an interagency meeting with the IAEA to review the implementation to date of the US/IAEA Safeguards Agreement. Other U.S. participants included the Department of State, ACDA, DOE, and NRC/IP.

The principal topic of discus-sion was the exchange of information between the U.S. and the IAEA, including problems the U.S. is encountering in the preparation and transmittal of accounting reports, and the ability of the IAEA to utilize the submitted information.

We have been informed that the IAEA has formally notified the U.S. Mission of the selection of the Combustion Engineering LEU Fuel Fabrication Plant at l

Windsor, Connecticut, Arkansas-2 reactor at Russelville, Arkansas, and the San l

Onofre-2 reactor at San Clemente, California, as replacements for the three i

licensed facilities currently under IAEA safeguards inspection.

The proposed penalty was based on a violation of license conditions in which unauthorized persons performed maintenance on the hospital's teletherapy unit, thereby compromising the safety of the unit.

The licensee responded to the Notice in letters dated April 19, 1983. After consid-eration of the licensee's response, the IE staff concluded that the violation occurred as set out in the Notice. Accord-ingly, an Order Imposing Civil Monetary Penalty in the amount of Two Thousand Dollars was issued.

The purpose of the training is to review NRC mission and emergency re-sponse procedures and familiarize the staff with the resources and equipment functions at the Operations Center.

Discussions were held last week with the INEL developers of RELAPS concerning the problems experienced by users at INEL and labs i

associated with NRR work. Many of the difficulties have been corrected in the MOD 2 version of the code which has now been completed.

Developmental assessment of the MOD 2 version will be completed in September 1983. Limited use will then begin in parallel with extended developmental assessment and independent assessment efforts. Those problems not corrected in MOD 2 will be corrected in MOD 1.5 and then added to MOD 2.

Many of the MOD 1.5 problems are from using a code which was not thoroughly assessed prior to its use. The MOD 2 version will receive an extensive checkout prior to release. The checkout and limited use with INEL experimental programishould result in correction of problem areas and allow for greater confidence in the code upon release.

2D/3D The Technical Coordination Committee / Steering Connittee meetings has decided that the large hole tie plate will be used in the UPTF test facility and that the development of data and algorithm should be the responsibility of FRG with the U.S. providing instrumentation for the testing facility and other support funds. The U.S. funds will come from reducing the number of instruments to be supplied.

Details will be decided at an analysis meeting to be held on July 18-19, 1983 at JAERI.

Dr. Zuber (NRC) and Dr. Chapin (MPR) will attend a meeting in Germany on July 8,1983 to discuss the correlations of the IDL data and the German redesign of the turbine meter position.

PBF As reported several weeks ago, the canned rotor pump in the PBF has been replaced. The pump has now completed operational checkouts and is declared ready for service. Parts (e.g., bearings) are on hand to repair the removed pump.

The Arrangement had been signed earlier by Mr. P. Tanguy, Director of the Institute of Protection and Nuclear Safety, CEA. Conclusion of this Arrangement culminated five years of mutual effort to reach an accord on this subject. The negotiations l

were complicated because of national proprietary interests in the breeder l

area and the complex relationships of the numerous foreign and domestic parties involved in the French breeder program.

After January 1, 1986, interregional agreements between siteless regions and States and those with operating sites were favored provided that the siteless regions implemented volume reduction and interim storage to the maximum extent feasible. Public participation and opposition was viewed as the major obstacle for obtaining new sites. Major recommendations were the following: The public needs to be brought into the siting process as effectively as possible and compact commissions or boards need to start moving forward on i

the siting process. Only the Northwest Compact appears to have incompatible definitions in its compact. The States feel any Federal LLW should be subject to the same disposal regulations as non-Federal LLW.

LLW generated by DOE and at DOE facilities should continue to be buried at DOE LLW sites. Also, any type of condition that Congress wishes to impose should be in the form of consent language for the individual compact rather than amending the 1980 law and care should be taken that the Congressional consent process does not require i

the compacts to be modified in State legislatures.

Finally, there was a recommendation that an interregional committee be established comprising the six compacts plus California, Texas and Kentucky under the aegis of the National i

Governors' Association and the National Conference of State Legislatures to discuss interregional issues in the future.

0FFICE FOR ANALYSIS AND EVALUATION OF OPERATIONAL DATA ITEM 0F INTEREST WEEK ENDING JUNE 24, 1983 Potential Contamination of the Spent Fuel Pool and Primary Reactor System An engineering evaluation was recently issued (AE0D/E313) which addressed failures of components due to stress corrosion cracking. These events included: Prairie Island 1 where there was a failure of the top nozzle in a fuel assembly and cracks in other fuel assemblies; Prairie Island 1 cracks in concentrated boric acid lines; and cracking in the TMI-1 steam generator tubes. The contaminant in the case of the fuel assemblies has not been identified. The contaminant in the other cases has been identified as sulfate.

Presently the water chemistry program for most PWRs does not routinely analyze primary coolant or spent fuel pool water for sulfates.

AE0D believes it would be prudent to analyze for such contaminates in PWR water chemistry programs.
