Narrative

Report for LER 50-321/1981-087 On September 2,

1981 while the eactor was operating at steady state power of 2430 MWT while the technicians were removing highly radioactive filter demins from the fuel pool, the refueling floor vent exhaust radiation monitor tripped.

This event caused a Group II isolation and resulted in isolating the (a) suction to the pumps connected to H2 and 02 analyzers and (b) drywell pneumatics.

The H2 and 02 analyzers became inop. and the drywell to torus delta p dropped below 1.5 psid.

This was a failure to comply with Tech Specs Section 3.7.A.6(c) and 3.7.A.7.

Neither plant operation nor public health and safety were affected.

This is a non repetitive event.

The cause of the event has been attributed to high radiation levels on the refueling floor.

The general area radiation levels were reduced by placing shielding (lead blankets and concrete filled dr ums), around the filter demins.

In the future, while performing any work involving high rad levels, health physics personnel have been cautioned to make careful estimates of radiation levels such that preventative methods, such as shielding, can be adopted to reduce these levels below the monitor trip setpoints.
