OYSTER CREEK NUCLEAR GENERATING STATICN Forked River, New Jersey 08731 Lic ssee Event Report Reportable Occurrence No.-50-219/81-33/0lT-0 Report Date August 6, 1981 Occurrence Date July 24, 1981 Identification of Occurrence A violation of Technical Specifications, paragraph 3.5.B.1 occurred when Secondary Contaiment Integrity was not maintained. Both personnel access contairrnent doors on the Northwest side of the Reactor Biilding were found ajar.

This event is considered to be a reportable occurrence as defined in the Technical Specifications,paragrapi6.9.2.a.2.

Conditions Prior to Occurren The plant was at steady state power.

Power: Reactor 1544 N t Generator 409 m e Flow: Recirculation 13.9 x 104 gpn Feedwater 4.71 x 10 lt/hr Description of Occurrence At approximately 1215 hours0.0141 days <br />0.338 hours <br />0.00201 weeks <br />4.623075e-4 months <br /> on July 24, 1981, the inside and outside (N.W.

corner, 23' elevaticn) Reactor Building personnel access air lock doors were found ajar. The Group Shift Supervisor was notified of the situation and upon arrival at the access air lock doors found than closed. The Group Shift Super-visor checked the interlocking systen and found it to be operating satisfactorily.

Electrical Maintenance was then requested to inspect the cccess door plunger latch mechanism.

Apparent cause of Occurrence Due to the continual opening and closing of the Reactor Building airlock doors, the interlock plunger mechanian was worn and had became loose. This created a mcznentary situation where the p'.unger muld not extend fully or engage p. ply, due to plunger mechanisn misalignment. Since the plunger had extended slightly, the interlock systen allowed the inside door to open, even though the outside Scor had not been locked closed.

Corrective Action Electrical Maintenan s tightened the plunger mechani m. A work order has been inai=d to replace the intarlN*'s worn parts, and an Engirmaring Task has been issued to evaluate, and possibly upgrade the Reactor Bi41 ding ' airlock door interlock systen. Due to the recurrence of this proble, a cartplete mechanical and electrical inspection will be conducted and an in=arliate refurbistunent effected to gain full usage of the intar1~ k systan to perfom its function.

This task will be assigned on a priority basis.
