Narrative

Report for LER 50-321/1981-013 On February 28, 1981, with the reactor in the shutdown mode, while performing the llPCI. Rated Flow Test to prove operability, the HPCI System was declared inoperable when an oil line near the rear bearing broke.

As the RCIC System

- was already inoperable, the requirements of Tech Spec 3.5.E.3 were complied with.

An investigation-into the cause of this incident revealed that the oil line had been damaged previously by someone stepping on the line.

The normal vibration of-the, turbine then caused the line to finish separating.

The broken oil line went to an oil -pressure gauge.

t The line was cut off, rethreaded and reconnected in the oil system.

The HPCI Rated Flow Test was then performed satisfactorily.

This was not a repetitive event nor were there any effects to public health and safety.

. Personnel were cautioned not to step on oil lines and the remaining HPCI and RCIC

. (~%, sil. lines; on both units were inspected for similiar damage. " No other problems A / were found.
