Dear Mr. Grier:

The following is provided for your information.

On December 9,1980, a routine review of station records by the Millstone Unit 2 Inservice Inspection Coordinator identified that some valve operability tests required by Reference 3 were not performed during the third calendar quarter 1980. - A detailed review of valve operations in this period indicated the following required tests were not documented:

68 of 159 valve excercising tests 34 of 91 required stroke time measurements 42 of 49 valve fail position tests The reactor plant was in cold shutdown for approximately half the test interval. During cold shutdown most of these valves are not required to be operable. Valves which were worked on during the cold shutdown were fully tested prior to start-up. All other valves have since been tested. None failed to meet acceptance criteria. Thus no valve failures went undetected.

Action has been taken to assure Inservice Testing of pumps and valves is performed when scheduled. In addition, action is being taken

- to assure more timely review of valve test data. These actions will prevent recurrence of this situation.
