In the performance of these examinations, Combustion Engineering, Inc. (C-E) acted as the prime contractor and supplied the examiners. Hartford Steam Boiler (HSB) served as the Authorized Inspection Agency. By this ) arrangement, the HSB Inspector reviewed the applicable procedures and I examiner's qualifications, witnessed the examinations and checked for general code compliance as specified in IM -2120 of Section XI. One hundred and sixteen (116) items were examined by the various exam-ination methods during this refueling outage. No reportable indications were found. O l I-l

Document No.: IR-ISI-010 Revision No.: 0 SUPNARY AND CONCLUSIONS The Fourth 'lefueling Outage ISI examinations of the Millstone Point, Unit 2 Power Plant were conducted during the interval from December 1981 through March 1982. The purpose of the ISI was to fulfill the require-ments of the 1974 Edition of Section XI of the ASME Boiler and Pressure Vessel Code, Paragraph IWA-2400. Selection of components to be inspected was in accordance with the 1974 Edition of Section XI including Addendas through the Summer of 1975. There were 4 types of examinations performed: Ultrasonic, Liquid Pene-trant, Visual and Eddy Current. These examinations were conducted by qualified personnel from Combustion Engineering, Inc. Written proce-dures were developed by Combustion Engineering and Northeast Utilities Service Company, and approved,by Hartford Steam Boiler Company and Northeast Nuclear Energy Company. Overall supervisory control of the program was supplied by personnel from the Nuclear Power Systems Develop-ment and Services Dept., Inservice Inspection Group of Combustion l Engineering, Inc. i Of the 116 items examined during the ISI, there were no reportable indi-cations discovered. The manual ultrasonic and eddy current procedures were developed by Combustion Engineering and approved by C-E, NNECo and HSB. The visual and liquid penetrant procedures were developed by Northeast Utilities and approved by NNECo and HSB. The ultrasonic search units (transducers and shoes) were varying types manufactured by KB Aerotech, Harisonics, Automation Industries and Search Unit Systems, Inc. Details of a particular instrument-search unit com-l bination used to perform a specific examination can be found on the f Calibration Data Sheets. I he eddy current instruments used for the RPV stud and nut surface exam-inations were M12-7's manufactured by Zetec, Inc. A. Discussion These examinations of Millstone Point, Unit 2, were conducted to fulfill the requirements of the Second 40 Month Inservice Inspection Interval. j B. General Examination Techniques The nondestructive testing was performed using ultrasonic, eddy current, liquid penetrant and visual techniques in accordance with written procedures required by IM -1400 of Section XI. Visual Examinations ne visual examinations were conducted on subject welds and compo-nents in accordance with Procedure No. NU-VT-1, Rev. 1. Liquid Penetrant Examinations , p) L A solvent removable type liquid penetrant examination was conducted j on subject welds in accordance with Procedure No. NU-LP-1, Rev. 1. l Ultrasonic Examinations All UT examinations were performed manually. The standard pulse echo technique was employed using 0 degree longitudinal, 45 degree shear and 60 degree shear waves. In addition to the standard pulse echo techniques, 60 degree and 70 degree shear waves were applied tan-gentially to the Pressurizer Surge Nozzle Inner Radii. Eddy Current Examinations Eddy current was performed as a surface examination on the RPV Studs and Nuts. l CE examination personnel performing the Inservice Examinations were certified in accordance with CE Procedures 00000-ESS-127, 00000-ESS-130, 00000-ISI-037 and 00000-ISI-036. These certification procedures are in l accordance with SNT-TC-1A, 1975 Edition. l O VIII-1

These colunns are titled (1) No Indications, (2) Insignificant Indica-tions, (3) Significant Indications, Geometrical, and (4) Significant Indications, Other. For this listing, all UT and ET indications with amplitude equal to or greater than reporting response were recorded, evaluated and reported in the tables. The term "No Indications" was used when the amplitude was less than the recording response er when no indications were observed. All recorded indications are considered significant with these caused by j component geometry listed as " Geometrical" and true indications listed lt as "Other".
