SUPPLEMENTARY INFORMATION Riport No.: 50-302/80-052/03L-0 Facility: Crystal River Unit #3 R: port Date: 5 January 1981 Occurrence Date: December 8, 1980 Identification of Occurrence:

Primary Coolant Dose Equivalent I-131 was greater than 1.0 microcuries per gram contrary to Technical Specification 3.4.8.a.

Conditions Prior to Occurrence:

Mode 3 (hot standby 0%).

Description of Occurrence:

Between 2030 and 2330 on December 7, 1980 Reactor power was de-creased from P4% to 0% for maintenance. The Reactor Coolant System was sampled at 2334 and the Dose Equivalent I-131 was determined to be within specificaticn. At 0730 on December 8, 1980 the RCS was sampled to investicate a higher than usual Gross Beta radioactivity result and the Dose Equivaler.t I-131 was de-termined to be 1.45 microcuries per gram at 0930.

Designation of Apparent Cause:

! The cause of this event is attributed to leaking fuel pins and an anticipated Dose Equivalent Iodine transient following the Reactor Coolant System transient of the previous night.

Analysis of Occurrence:

Redundancy N/A. There was no effect upon the health or safety of the general public.

Corrective Action:

The Reactor Coolant System was recirculated through the Makeup and Purification System; Dose Equivalent I-131 was determined to be less than 1.0 microcurie per gram at 2230 December 8, 1980.

Failure Data:

This is the nineteenth occurrence reported under this specifica-tion.

e ITEM 5 As per Technical Specification 3.4.8, the four (4) hour sampling frequency as depicted on the table below was initiated at 0930 on 12/8/80 and the Dose Equivalent I-131 was 1.45 microcuries per gram. The four hour sampling frequency was terminated at 2230 on 12/8/80 when the DEI-131 was determined to be .386 microcuries per gram. The time duration when the specific activity of primary coolant exceeded 1.0 micrucuries per gram Dose Equivalent is fourteen (14) hours.
