The Fort Calhoun Station Unit No.1 Engineered Safety Feature System is so designed that no single failure can prevent the performance of a safety function if required.

During the time that the 86-Al/CPHS (Containment Pressure High Sig-nal Lock-out Relay) was de-energized and thus inoperable, the 86A/CPHS i

(A Channel), tha 86-B/CPHS (B Channel) and the 86-Bl/CPHS (B Channel l

Cerived Signal) relays were all operable and available to mitigate the l

consequences of an accident if required.

Corrective Action

The cause of the pcwer failure to the 86-Al/CPHS relay, (containment Pressure High Signal Channel A Derived Signal Relay), was a dirty set of i

contacts on the Channel "A" derived signal cutoff switch CS/SP-A.

Therefore, the contacts of the switch were cleaned and burnished.

Sub-sequencly, the derived signal cutoff switch and the asscciated 86-Al/

CPHS lockout relay were returned to satisfactory / normal operation.

l This is the first failure of a derived signal cutoff switch contact which caused a loss of power to an Engineered Safeguards lockout relay at the Fort Calhoun Station.
