Corrective Action

"A" charging pump. weld of the flush line was repaired by grinding i

- out the old weld and rewelding using a certified procedure. The line l

was inspected and hydrostatically tested before returning to service.

Pulsation ' dampeners were. installed on "B" and."C" charging pumps in l

1979. Weld fractures have not been a problem on the piping asscciated with these pumps since the modifications. A pulsation dampener was recently installed on "A" _ charging pump.

Based on the performance of the other charging pumps with pulsation dacpeners installed, future problems of this type are not anticipated, j
