ATTACHMENT TO LICENSEE EVENT REPORT NO. 80-013/OlT-0 Wisconsin Electric Power Company Point Beach Nuclear Plant Unit 1 Docket No. 50-266 At approximately 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br /> on November 13, 1980 it was discovered that the suction to the IP4B boric acid transfer pump was isolated, thus rendering the pump inoperable. The isolation of the pump probably occurred due to personnel error in returning the valve lineup to normal following recirculation of the "B" concentrated boric acid tank for Technical Specification sampling on November 10 at approximately 1230 hours0.0142 days <br />0.342 hours <br />0.00203 weeks <br />4.68015e-4 months <br />. For a portion of the postulated time that the IP4B pump was inoperable, the 2P4A pump was out-of-service for maintenance. The 2P4A pump was returned to service at 1215 hours0.0141 days <br />0.338 hours <br />0.00201 weeks <br />4.623075e-4 months <br /> on November 11. During the time period of 1230 hours0.0142 days <br />0.342 hours <br />0.00203 weeks <br />4.68015e-4 months <br /> on November 10 and 1215 hours0.0141 days <br />0.338 hours <br />0.00201 weeks <br />4.623075e-4 months <br /> on November 11 both units were operating with two out of four boric acid transfer pumps out-of-service. This mode of operation is allowed by Technical Specification 15.3.2.D.2 for a 24-hour period. However, since the two pumps were out-of-service (one unknowingly) for a period of time that_can only be-estimated, this event is considered a vioation of the limiting condition of operation contained in 15.3.2.D.2 of the Technical Specifications.

4. The pump was returned to service immediately upon discovery by correction of the valve lineup by the auxiliary operator. The correct system lineup was verified by the duty shift supervisor.

Operability of the pump was also verified.

At all times each unit had two sources of borated water available for injection through three paths. The charging pumps had concentrated boric acid available through the remaining two boric acid transfer pumps and refueling water (>2000 ppm boron) from their direct suction. The safety injection pumps which take

their suction directly from the boric acid storage tanks or'the refueling water storage tank were also available.

The boric acid transfer pumps are not part of an automatic system used to mitigate the consequences of an accident. Manual action is required to inject high concentration boron via the boric acid transfer pumps and charging pumps. Automatic injection of high concentration boron is accomplished by the safety injection pumps which take their suction directly from the boric acid storage tank. The capability of automatic injection of high concentration boron was not impaired by the shut valve in the suction of the transfer pumps.

e The Technical Specification requires that the boric acid storage tanks be sampled twice a week. Although the valve lineup for this operation is simple, to prevent a future occurrence of this event a checklist for valve operation is being prepared to verify proper valve lineup following sampling of the boric acid storage tanks.

The resident inspector was notified on November 13.

This event is reportable in accordance with Technical Specification 15,5.9.2.A.2 and is the first occurrence.
