On October 6,1989, at approximately 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br />, in Mode 1 at 100cc power, Operat:ons Department (OD) shift personnel discovered that all Unit 3 hydrant hose houses (HHHs) did not have the required fire hose lengths. Shift personnel had remosed fire hoses from 3 of 11 HHHs on October 2, M9 to support a non-fire related emergency. During performance of a hose house flush surveillance on October 6, the

' deficiency was initially identified for the 3 HHHs. Further investigations determined that all HHHs did not have the required hose lengths.

[ Root causes of this event were personnei error and procedural deficiency. OD personnel removed fire hoses on October 2. without taking compensatory action. The Building Services Department "HHH" surveillance performed on October 5. identified the deficiency in the 3 HHHs and incorrectly evaluated the

  • inventory in the remaining 8 HHHs. The procedure did not require immediate notification of the Shift Supervisor (SS) upon discrepancy identification.

l - Immediate corrective action was to restore all HHHs to operable status. A memo was issued to all OD shlit personnel prescribing the usage requirements for HHH equipment. Caution signs will be posted on all l .- HHHs by November 30,1989. The applicable surveillance procedure has been revised to require a Senior l Licensed Operator review of procedure revisions and immediate notification of the SS upon discrepancy identification.

e L Operations Department shift personnel, all Unit 3 hydrant hose houses were found to have insufficient lengths of fire hose available to fulfill hydrant hose house operability requirements. On October 6.1989 at approximately 1815 hours0.021 days <br />0.504 hours <br />0.003 weeks <br />6.906075e-4 months <br />, additional lengths of fire hose were installed to fulfill

hydrant hose house . operability requirements.

The unit staff failed to fulfill the requirements of Technical Specification Limiting Condition of f Operation (LCO) 3.7.12.6.a. " Yard Fire Hydrants and Hydrant Hose Houses". The durution of this event was approximately 4 days. .

On October 2,1989 Operations Department shift personnel removed lengths of fire hose from three hydrant hose houses to support a no1-fire related emergency in the Unit 3 Circulating Water Intake structure. Shih personnel fa .ed to supply the three hydrant hose houses with supplemental hre hose to ensure hydrant hose he.ase operabihty.

11. Came of Event The root causes of the event were >oth personnel error end procedural deficiency. Shift personnel removed lengths of hre hose from bree of the eleven Unit 3 hydrant hose houses without supplying

I Building Services Department persor.nel inadequately evaluated the number of lengths of 11/3 inch fire hose in eight of eleven Unit 3 hydrant hose houses. The apphcable Building Services i ' procedure indicated the proper length of fire hose but also referred the user to an unauthonted operator aid located in each hydtt.nt hose house. The unauthorized operator aid indicated the incorrect length of fire hose.

This event is reportable pursuant to 10CFR50.73 (a)(2)(i), as a condition prohibited by the plants Technical Specification.

Each hydrant hose house is required to have 130 feet of 1 '/g inch fire hose. While only selected hydrant hose houses require 150 feet of 1 1/2 trich fire hose to perform their required function, all hydrant hose houses were inventoried with identical lengths of 1 '/g inch fire hose to ensure commonality of all hydrant hose houses.

Fire suppression for the affected areas consists of the unit fire brigade. With the absence of a portion of the required inventory of hre hoses in the hydrant hose houses tire suppression capabilities become degraded. Fire protection hoses from associated hydrant hose houses which

' were in close proximity were available had a fire occurred in the aflected areas. Therefore, there were no significant safety consequences as a result of this event.

IV, Corrective Action immediate corrective action was to restere all hydrant hose houses to operable status and the unauthorir.?d operator aids were removed from cach hose house. This was accomplished by F ensuring all Unit 3 hydrant hose houses were supplied with 150 feet of 1 '/ inch fire hose, As interim corrective action, a memorandum was issued to all Operations Department shift personnel stating that hye: rant hose house equipment shall be used for fire fighting acthities only, unless an emergency condition exists, The memorandum states the Operations Department Shift Supervisc,7 must be notified pnor to removal of fire hoses. As action to prevent recurrence, siens will be posted on each hydrant hose house statmg, "For Fire Brigade Use Only, Notify Shth Supervisor -

! November 30, 1989. The applicable Buildmg Services Department surveil:ance has been revised to i require a Senior Licensed Operator review future procedure revisions pnor to being presented to the Site Operations Review Committee (SORC).
