LER No.80-016 Omaha Public Power District Fort Calhoun Station Unit No.1 Docket No. 05000295 ATTACHMENT NO. 1 Safety Analysis During the time that CH-1B was out of service for repair of the crack in the demineralized flush line, CH-1A and CH-1C were operable except for a two hour period that CH-lC was out of service due to a solenoid that failed open on the leak off line.

LER No.80-016 Omaha Public Power District Fort Calhoun Station Unit No.1 Docket No. 05000285 ATTACHMENT NO. 2 Corrective Action The crack in the weld of the demineralized flush line was repaired by grinding out the old weld and rewelded using a certified procedure.

The line was hydrostatically tested before returning to service.

The vibration in the piping has been addressed and a modific.ation to install suporessors on the pump suction and discharge side has tieen partially completed.

Incidents that are related to this failure were previously documented on LER 78-005,78-006 and 78-046.
