3 ATTACHMENT - LER 80-18/lX-0 Millstone Unit 2 Docket No. 50-336 EVENT DESCRIPTION: During the on-going hanger base-plate analysis for NRC IE Bulletin 79-02, it was determined that several se'rvice water support hangers did not provide a factor of safety greater than two. Therefore, both service water headers were declared inoperable which resulted in plant operation per Technical Specification 3.0.3 since conditions were in excess of those addressed in the Action Statement of Technical Specification 3.7.4.1. Accordingly, preparations were made to Proceed to place the plant in cold shutdown within 30 hours. With the plant in cold shutdown for service water support repairs, it was determined on 5/29/80 that several hangers on the A and B Reactor Building Closed Cooling Water (RBCCW) headers did not provide a factor of safety greater than two (2). Both RBCCW headers were declared inoperable. CAUSE DESCRIPTION AND CORRECTIVE ACTION: , The cause of service water and RBCCW headers declared inoperative ivas 'etermination that presently installed hangers did not provide a factor of safety greater than, two (2). Since 5/9/80, the plant has been in cold shutdown as required per Technical Specification 3.0.3. A pipe stress analysis run with the effected hangers removed showed the potential for piping overstress conditions. Work commenced 4 in parallel with the analysis of additional systems identified for further review under 79-02 to upgrade all effected hangers. Redesign and modification of service water, RBCCW and other safety related System supports with a factor of safety less than two (2) was completed on 6/21/80.
