800430o APPENDIX A NOTICE OF VIOLATION Florida Power Corporation License No. DPR-72 Crystal River, Unit 3 Based on the NRC inspection July 2 through August 3,1979, certain of your activities were apparently not conducted in full compliance with NRC require-ments as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.

A.

As required by 10 CFR 20.203(c), each entrance or access point to a high radiation area shall be equipped with a control device and a conspicuous visual or audible alarm. Radiation Control Procedure, RP-101, Radiation Protection Manual, Section 4.2.2.a states in part that, "in lieu of the control device or alarm signal, each high radiation area in which the intensity of rad.lation is greater than 100 mrem /hr. but less than 1000 mrem /hr. shall be barricaded and conspicuously posted as high radiation area...".

Contrary to the above, on July 8,1979, on two separate occasions, the entrance to the block orifice room in the auxiliary building was posted as a high radiation area but not barricaded.

This is an infraction.

B.

As required by Technical Specification 6.8.1.a, procedures referenced in Regulatory Guide 1.33 (1972), Appendix A, Section A.3, shall be implemented in the area of equipment control. Plant Clearance Procedure, CP-115, "Inplant Equipment Clearance and Switching Orders", requires that the equipment / system to be tagged will be identified by the complete name of the equipment / system on the clearance log. Enclosure 1A is an example of the correct method for completing the clearance log. The equipment / system will not be identified by number only.

Contrary to the above, on July 26, 1979, several of the active clearance logs examined were not completed as required in that the equipment / system cleared and tagged for maintenance was identified by number only.

This is a deficiency.
