EVENT DESCRIPTION

On March 16, 1980 at 1400, wi th Uni t Two shutdown for refuel, special General Electric Procedure TP-508.0642 was performed to inspect Recirculation Water Jet Pumps I through 20.

This procedure provides for ultrasonically examining jet pump fixture beam assemblies.

During the course of this inspection, the beam fixture on jet punp number 16 was discovered to have a width-wise crack indication along the upper machined surface located near the beam bolt. None of the other 19 beam jet pump assemblies showed any indication of cracks.

Work request number Qo4198 was written to replace the beam fixture.

A new beam assembly on jet pump number 16 was installed by March 24.

VI.

PROBABLE CONSEQUENCES OF THE OCCURRENCE:

The jet pump beam fixture is designed to wi thstand the combined loadings from differential pressure and temperature, fluid movement, and vibration wi thout failure or loss of integrity. The affected fixture did not lead to jet pump inoperability of loss of integrity.

The jet pump hold-down components maintained the capabili ty to perform their design function. Thus, there were no unsafe conditions during previous periods of reactor power operation based on this occurrence.

CAUSE

An investigation of this jet pump beam fixture and cracked jet pump beam fixtures f rom the Dresden Station is being performed at the General Electric Company Vallecitos Nuclear Center. The cause of the crack has been identified as intergranular stress corrosion cracking (IGSCC).

CORRECTIVE ACTION

t The complete ultrasonic inspection of the Unit Two jet pump assemblies and replacement of the cracked beam assembly is deemed adequate immediate corrective action. A similar ultrasonic I

inspection will be performed on the Unit One jet pump assemblies during the next scheduled refueling outage in the fall of 1980.
