Identification of Occurrence:

Environmental samples were not taken from locations at the frequen-cies required or were not analyzed using procedures which provided LLDs equal to or less than those required by Environmental Techni-cal Specification 3.2.2.

Conditions Prior to Occurrence:

N/A 7.

Description of Occurrence:

1.

The air particulate filter for Station C41 for 2 November,

- 1979, was not transmitted to the University of Florida for weekly Gross 3 and quarterly composite gamma spectral analysis.

2.

The 5 November,1979, air sample for Station C46 was not ob-tained for weekly Gross 8 and quarterly composite gamma spec-tral analysis.

3.

The August,1979, precipitation samples for Stations C04 and C40 for tritium analysis were inadvertently spilled by the contractor to whom they were sent and, consequently, the monthly analyses could not be performed.

4.

The following samples had ! LDs greater than that raquired for the periods indicated-below:

Air Particulate (7 stations)

Fourth Quarter

Description of Apparent Cause:

Referenced to the Description of Occurrence above:

1.

Failure to transnit the appropriate air particulate filter was due to personnel error.

2.

Failure to obtain the appropriate air sample was due to personnel error.

3.

Spillage of the samples was an accidental occurrence during the State of Florida's relocation of their facilities to a new laboratory.

4.

The air particulate filter from Station C41'for 2 November, 1979, was not transmitted to the University of Florida for analysis.

High LLDs for Ce-144, Th-232, and K-40 for the Fourth Quarter for Station Cal are apparently due to the decreased sample volume resulting from loss of the filter and failure of the person changing the sample to calibrate the flow to one cubic foot per minute.

It is not uncommon for certain uncontrolled circumstances to prevent achieving the required sample LLD-from time-to-time.

Apparently, the high LLDs for I-131 for :the Second Half for Stations C29 and C30 are the result of such circumstances.

Analysis of Occurrence:

' Referenced to the Description of Occurrence above:

1,2,3. These occurrences did not result from operation of Crystal River Unit 3, nor is operation of the plant affected by these occurrences.

4.

The Gamma activity. associated with Ce-144 at Station C41 for air particulate in the Fourth Quarter was nondetectable.

The gamma activity in the Fourth Quarter associated with Th-232 and K-40 was 0.140 pCi/M3 and 0.120 pC1/M3, respectively.

The. Gamma activity associated with I-131 at Stations C29 and C30 for oysters in the Second Half.was nondetectable.

This occurrence did not result from operation of Crystal River Unit 3, nor is operation of the plant affected by this occurrence.

Corrective Action

Referenced to the Description of Occurrence above:

1,2.

Since the incident there has been no recurrence of this event.

These occurrences are' infrequent personnel errors, and, as such, corrective actions are not necessary.~ Procedures in effect at the time of the occurrences are adequate for control of these sampling activities as no similar occurrences have since occurred.

This occurrence was an event during the relocation of the State of Florida's facilities, and, as such, no corrective actions are necess ary.

4.

The LLD for Ce-144 from Station C41 for air particulate (gamma 3

analysis) in the Fourth Quarter was 0.160 pC1/M.

The LLD for 3

3 Th-232 was 0.030 pCi/M.

The LLD for K-40 was 0.024 pCi/M,

The LLDs for I-131 from Stations C29 and c',u

",r oysters (gamma analysis) in the Second Half were 12 pCi/K? and 13 pCi/KG, respectively.

Analyses are performed in such a manner that the required LLDs will be achieved under routine conditions.

Occasionally, back-ground fluctuations, unavoidably small. sample sizes, the presence of interfering nuclides, or other uncontrollable circumstances may render these LLDs unachievable.

The 1979 annual average LLDs for air particulate (gamma analysis) associated with Ce-144,3 Th-232, and K-40 were 0.054 pC1/m3, 0.016 pC1/m3, and 0.085 pCi/m respectively.

These values are less than the respective required LLDs.

The 1979 annual average LLD for I-131 Stations C29 and C30 for oysters. (gamma analysis) was 9pCi/KG which is less than the required LLD.

As a result, no corrective action is proposed.
