FITZPATRICK NUCLEAR POWER PLANT DOCKET NO. 50-333 ATTACilMENT TO LER 80-013/03L-0 Page 1 of 1 During normal operation, while conducting operations Surveillance Test F-ST-22C titled " ADS System Logic Functional Test," reactor low-low-low water level switch 02-3-LIS-72A did not operate properly. Level switches 02-3-LIS-72B, C and D were verified as operating properly and within the limits of Technical Specifications Tabic 3.2-2 therefore, the event did not represent any significant hazard to the public health and safety.

Investigation revealed that the level switch contact'did not properly close at the switch set point. Following replacement of the switch with a new switch surveillance was increased from once each month to once each week to provide assurance of proper operation. One week late (on January 21, 1980) it was discovered that the new switch caused a ground on the plant's 12S volt DC system when the switch was at set point. This ground was caused by contact between the instruments indicator magnet and a switch contact terminal. In order to climinate this ground, instruments 02-3-LIS-72B, C and D were verified operabic by performing Instrument Surveillance Procedure F-ISP-3-2 titled

" Reactor Low-Low-Low Water Level Instrument Calibration." Table 3.2-2 also requires that when one (1) of the four (4) instruments of concern is made or found inoperable the associated system shall be placed in the tripped condition or the reactor placed in the cold condition within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />.

Administrative review of the circumstances associated with replacement of 02-3-LIS-72 A on January 21, 1980 (due to the grounding problem) revealed that during the time required to replace the instrument the system was not in the tripped condition. This occurred, in part, due to the short time required to replace the instrument (approximately ten minutes). In addition, the shift Supervisor did not recognize when he authorized Instrument G Control personnel to proceed with replace-ment of the instrument that the work would be initiated almost immediately. Prior to granting authorization for replacement the instrument channel should have to be tripped. To prevent recurrence the Operations Superintendent discussed the event with the Shift Supervisors and stressed the importance of not granting work authori-zation until all of the required actions of Technical Specifications have been met.

Following replacement of 02-3-LIS-72A proper operation of the switch was demonstrated by completion of F-ISP-3-2.
